Efficacy of Intravenous Cyclophosphamide Therapy for Neuromyelitis Optica Spectrum Disorder

Abstract: Objective Neuromyelitis optica (NMO) is an inflammatory disease that affects the optic nerve and spinal cord. Optic neuritis and longitudinally extensive myelitis associated with systemic autoimmune disease have been recently defined as NMO spectrum disorder (NMOSD). In this study, we report the efficacy of intravenous cyclophosphamide (IVCY) therapy for NMOSD. Methods Four patients diagnosed with NMOSD were enrolled in this study. The expanded disability status scale (EDSS) score was used to evaluate the degr… Show more

“…This drug has been evaluated in NMO in two small clinical case series, with differing results. One small study of four patients with NMOSD reported a reduction in median EDSS score from 8.0 to 5.75; 72 by contrast, no improvement in relapse rate or disability was observed in another study of seven patients with NMO. 73 …”

“…In one series of ten subjects with NMOSD unresponsive to corticosteroids± PLEX, IVIg was helpful in five [16]; this series prompted a larger, multi-center randomized controlled study in NMOSD and acute TM [17]. Small case series also support the use of cyclophosphamide in acute steroid/PLEX-unresponsive NMOSD attacks, especially in the context of systemic lupus erythematosus (SLE) and other systemic autoimmune diseases [18, 19]. …”
