Efficacy of Everolimus for Treating Renal Angiomyolipoma with Inferior Vena Cava Thrombus Associated with Tuberous Sclerosis: A Case Report

Abstract: Here we report a case of 57-year-old woman with renal angiomyolipoma associated with tuberous sclerosis complex involving inferior vena cava thrombus. We could perform less invasive nephrectomy with thrombectomy because everolimus administration reduced the inferior vena cava thrombus. To the best of our knowledge, this is the first report the use of everolimus before performing surgery to treat renal angiomyolipoma with inferior vena cava thrombus.

“…Several case reports showed the efficacy of everolimus even for a large AML. 2 In TSC patients, 85% of them have TSC1 and TSC2 genetic defects, leading to the deficiency of a protein that regulates mTORC1 activation. 3 Everolimus directly inhibits mTORC1 and disrupts TSC-AML cell growth through the mTOR pathway.…”

“…There is no established treatment for unresectable cases; however, mTOR inhibitors have been reported to be effective in AML associated with tuberous sclerosis. [4][5][6][7][8][9] There have also been reports of responses to tyrosine kinase inhibitors in some cases. [7][8][9][10][11][12][13] Unlike RCC, intravenous extension to inferior vena cava (IVC) is extremely rare in EAML.…”
