Efficacy of emergency laparoscopic appendectomy in treating complicated appendicitis for elderly patients

Abstract: Objective:To assess the therapeutic effects of emergency laparoscopic appendectomy (LA) in treating complicated appendicitis (CA) for elderly patients (defined as age >65 years). Methods:We conducted a retrospective study of 115 elderly patients with CA who underwent surgical therapy in the Affiliated Huizhou Hospital of Sun Yat-Sun University, Huizhou, Guangdong Province, China between September 2014 and August 2016. Of these, 59 patients consented to open appendectomy (OA), and LA was performed in the other 5… Show more

“…In Garg's study, 110 patients presented with CA and were operated on [20]. Similar results in favor of the laparoscopic method, also without statistical significance, are reported by Quezada (2.1% vs. 6.9%), Wu (5.4% vs. 20.3%), Mohamed (1.5% vs. 3.6%), Minutolo (1 vs. 3 patents) and Horvath (0.17% vs. 0.5%) [11,21,[22][23][24]. To the contrary, Lim reports a higher incidence of postoperative ileus in the laparoscopic group of patients presented exclusively with periappendicular abscess (16.7% vs. 8.3%; p = 0.028) [25].…”

“…Only when the patient is reluctant to undergo surgery or the patient's general condition is poor, combined with multiple organ failure and other surgical contraindications can we consider conservative treatment. CA can result in a variety of potentially serious complications, such as peritonitis, abdominal abscess, and paralytic intestinal obstruction, [23] and its surgical treatment should be performed as early as possible and adequately prepared before the operation, in order to reduce intraoperative and postoperative complications, and reduce mortality. [13,24] Particularly for elderly patients, they have less physiologic reserves and poorer nutritional status, as well as more postoperative complications and mortality, when compared with younger patients.…”

“…[13,24] Particularly for elderly patients, they have less physiologic reserves and poorer nutritional status, as well as more postoperative complications and mortality, when compared with younger patients. [23] These fully illustrate the importance of immediately determining whether the patient has CA before the operation, in order to prevent the delayed timing of surgery due to complicated preoperative preparation or any affect on the prognosis of patients due to the choice non-surgical treatment. The study aims to identify whether it is CA as soon as possible after admission, so as to select the optimal treatment promptly, decrease unnecessary examinations and treatment, and reduce related complications.…”
