Efficacy of DynaCT Digital Angiography in the Detection of the Fistulous Point of Dural Arteriovenous Fistulas

Abstract: BACKGROUND AND PURPOSE:Identifying the precise hemodynamic features, including the fistulous point, is essential for treatments of dural arteriovenous fistulas (DAVFs). This study illustrates the efficacy of DynaCT digital angiograms obtained from a 3D C-arm CT to directly visualize the location of the fistulous points in DAVFs.

“…In fact, experience in evaluating DAVF with C-arm CT, as recently described by Hiu et al 17 and Lasjaunias et al, 33 helps generate confidence in evaluating CTA correlates of classic angiographic findings. 32 The current series supports findings of earlier reports that suggest that DAVFs associated with PT often involve the sigmoid sinus.…”

“…Various imaging strategies have been proposed, including MR imaging, MRA, MRV, carotid sonography, CTA/CTV, and DSA. [12][13][14][15][16][17] None of these studies have well-defined test characteristics. Conventional angiography remains the most sensitive and specific technique for diagnosing DAVF.…”

“…31 Spatial resolution is lower on images of 3T TOF MRA than DSA, and 3T TOF MRA does not provide the temporal resolution of the vascular flow dynamics. Consequently, information of 3T TOF MRA regarding venous anatomy is inferior to that of DSA, and 3T TOF MRA cannot replace DSA at the treatment planning stage.…”

“…[14][15][16][17][18][19] It is, nonetheless, possible that slow-flowing CVR or fistulas may be missed by using these techniques, given the lack of selective arterial injection and lesser temporal resolution compared with DSA. This situation has recently been encountered in a series of DAVFs evaluated by dynamic CTA 14 and highlights the persisting role of conventional angiography in DAVF characterization.…”
