Efficacy of daptomycin in the treatment of enterococcal endocarditis: a 5 year comparison with conventional therapy

Abstract: Daptomycin-treated patients more frequently required a therapeutic change due to worse microbiological and clinical response, although mortality was not increased. Our findings do not support the use of daptomycin as single therapy in the treatment of EIE. Its role in combined strategies should be further investigated.

“…The rationale for why higher doses of daptomycin may be required for the optimal treatment of such serious infections is based on the observation that daptomycin MICs for Enterococcus species are typically higher than those for other Gram-positive organisms (0.5-4 vs 0.25-1 mg/L) [ 170 ]. On the other hand, there have been reports of unsatisfactory outcomes of daptomycin monotherapy for serious enterococcal infections [ 171 ] suggesting that daptomycin may need to be combined with other antimicrobials for clinical cure, although clinical evidence to support this hypothesis is currently limited.…”

“…Other series have shown worse results [120,124]. In a recent ICAAC, Ceron et al presented a 5-year study on daptomycin efficacy for enterococcal IE, with promising results [184]. Some successfully treated cases of multidrugresistant E. faecium endocarditis with daptomycin plus tigecycline combination have been reported [130,[134][135][136].…”
