Efficacy of Commercially Available Topical Cyclosporine A 0.05% in the Treatment of Meibomian Gland Dysfunction

Abstract: Topical CsA may be helpful in the treatment of meibomian gland dysfunction (posterior blepharitis). Topical CsA did not induce an improvement in the symptoms, but it did decrease the number of meibomian gland inclusions in patients with meibomian gland dysfunction.

“…1,2 This is supported by the fact that cyclosporine A, a highly potent immunosuppressive agent, improves some of the signs of posterior blepharitis. 28,29 Notably, topical administration of cyclosporine A emulsion has been shown to improve the quality of MG secretion and decrease the number of MG inclusions (plugging) after 2 months of therapy. 28,29 Clearly, both cyclosporine A and azithromycin act to ameliorate MG abnormalities, although their modes of action are different: cyclosporine A affects primarily T lymphocytes, while neutrophils may be the major leukocytic target for azithromycin.…”

“…This is well documented in a recent report by Jang et al 7 It has been suggested that meibomian gland dysfunction can be treated effectively with cyclosporine A 0.05%. [22][23][24] The ocular prosthesis causes mechanical trauma to the tarsal conjunctiva, thus altering the meibomian gland structure. 3,25 Furthermore, poor hygiene with the prosthetic eye can lead to inflammation of the anophthalmic socket.…”
