Efficacy of Combination Use of Beta-Lactamase Inhibitor with Penicillin and Fluoroquinolones for Antibiotic Prophylaxis in Transrectal Prostate Biopsy

Abstract: PurposeTo investigate the efficacy of tazobactam/piperacillin (TAZ/PIPC) plus levofloxacin (LVFX) as a prophylactic administration in transrectal prostate biopsy (TPBX).Materials and MethodsWe investigated 201 consecutive patients who underwent TPBX in one Japanese hospital during the period of 2009-2010. The patients received TAZ/PIPC 4.5 g i.v. once just before and 3 hours after TPBX, plus oral LVFX 300 mg or 500 mg daily for 3 days. We examined the infectious adverse events and laboratory data (serum white … Show more

“…13 In the present study, we could not perform any temporal analyses of complications because the length of the study was insufficient; to the best of our knowledge, there have been no such temporal analyses in Hong Kong. The infection rate in our cohort was comparatively lower than that of most international studies, 4 and similar to that in prior studies elsewhere in Asia 14,15 (0% and 0.5% of PBI), as well as in Hong Kong 5,6 (0.5% and 3.9%). Reasons for the apparent lower infection rate in people of Asian ethnicity compared with those of other ethnicities are unclear.…”

“…Taken together, AMP with a single high dose of quinolones is recommended for low‐risk patients who are undergoing a transrectal prostate biopsy (EL, II; RG, B). Additionally, a combination of aminoglycosides with quinolones or tazobactam/piperacillin can reduce the rate of infection, and is considered to be an optional selection (EL, IVa; RG, C1). As for AMP for high‐risk patients who have a large prostate volume of 75 mL or more, diabetes, steroid dosing, voiding dysfunction (IPSS ≥20, maximum urinary flow rate ≤12 mL/s, residual volume ≥100 mL) or immune‐suppression status, tazobactam/piperacillin at 4.5 g, twice for a single day is recommended (EL, III; RG, B) …”
