Background: Previous studies reported the beneficial effects of walking in individual with mild to moderate knee osteoarthritis (OA). The current study aimed to compare the effect of 6-week retro versus forward walking program versus control group on pain, functional disability, quadriceps muscle strength and physical performance in individuals with knee OA. Methods: A three-arm single-blinded, randomized, controlled trial and intention-to-treat analysis was conducted in outpatient physiotherapy department, King Saud University, Saudi Arabia. Sixty-eight individuals (mean age, 55.6 years; 38 female) with knee OA participated. The participants in the retro or forward walking group completed 10 min of supervised retro or forward walking training in addition to usual care, 3 days/ week for 6 weeks. The control group received a routine physiotherapy program. This program comprises a combination of closed and open kinematic chain exercises, including straight leg raising, isometric quadriceps, isometric hip adduction, terminal knee extension, semi-squat, and leg press. The primary outcomes were mean pain and knee function score measured by the numerical rating scale and the Western Ontario and McMaster Universities Osteoarthritis Index, respectively. The secondary outcomes were mean score of quadriceps muscle strength and timed up and go test scores. All the outcomes were analyzed at baseline and week 6.

Objective In middle-aged individuals with early-stage knee osteoarthritis (OA), follow-up with remote rehabilitation methods may provide effective clinical outcomes. The present study aimed to investigate the effect of telerehabilitation (TR) on pain, function, performance, strength, proprioception, and psychosocial parameters in patients with knee OA. Design A randomized controlled study was conducted with 54 participants with knee OA. All patients were randomized into two groups: the home-based telerehabilitation group (TRG) (n = 29) and the home-based paper group (PG) (n = 28). The home exercise program and training were provided with an instruction manual to PG. The same home exercise and education prescriptions were given to individuals in TRG via an online platform. Pain, function, quality of life, physical performance, muscle strength, and proprioception were evaluated at baseline and after eight weeks of intervention. In addition, satisfaction and usability were evaluated at week 8. Results TRG demonstrated better improvement in terms of pain, function, quality of life index score, and left extremity proprioception (p < 0.05). TRG was not superior to PG in terms of muscle strength and performance tests (except the Five Times Sit to Stand Test) (p > 0.05). Conclusions The home-based TR program was superior to the home-based PG program for pain, function, quality of life, and proprioception in patients with knee OA.

Background Osteoarthritis (OA) is a chronic, degenerative, inflammatory musculoskeletal dysfunction. It is associated with pain, reduced functional capacity, and deficient quality of life posing a challenge for rehabilitation. The study objective was to determine the treatment protocol for effectively managing pain and functional disability of patients with OA knee. Methods This is a single-blinded study conducted in the outpatient department of a tertiary care hospital. This three-arm study consisted of 20 patients randomly allocated to each group, prearranged to receive conventional treatment (group A), conventional treatment with retro walking training (group B), and conventional treatment with balance training (group C) for a time frame of 6 weeks. Patients were screened prior to the pre-set eligibility criteria before inclusion in the study. Pain and functional disability were assessed at the baseline and post-intervention. Results The findings reported 66.66% female participation. The results established statistical significance for both pain and functional disability across all the three groups for both intergroup and intragroup analysis. Group B demonstrated the most promising results. Conclusion The 6-week rehabilitation protocol comprising retro walking as an adjunct to conventional therapy resulted in greater pain reduction and enhanced functional performance of patients with OA knee. This benefits the community at large. All stakeholders should be encouraged to include retro walking in their routine life. Trial registration The University Hospital Medical Information Network Centre (UMIN-CTR), UMIN000044483. Retrospectively registered on June 09, 2021.
