BackgroundTo determine if alveolar vertical distraction osteogenesis obtains better results compared to other bone regeneration treatments (guided bone regeneration with membranes and / or filling material, or autogenous bone graft) in terms of bone gain, complications, and implant survival and success rates.Material and MethodsAn electronic search was performed in Pubmed (MEDLINE), Cochrane Library and Scopus databases in March 2017. Besides, a manual search was carried out. Inclusion criteria were randomized controlled trials published within the last 10 years with at least 1 year of follow-up after implant placement. No language restriction was applied. Exclusion criteria were studies in patients with bone defects produced by trauma, congenital malformation or oncologic surgical treatment. The methodological quality of the selected studies was evaluated by means of the Cochrane Collaboration’s Tool for assessing risk of bias. The reports were classified into different levels of recommendation according to the “Strength of Recommendation Taxonomy “.ResultsOut of 221 articles, two randomized controlled trials were finally selected for the inclusion in the systematic review. Bone gain and complications were higher with the alveolar vertical distraction osteogenesis compared to the autologous bone graft. There was higher bone resorption with the autologous bone graft. Implant survival and success rates were similar between studies, despite of the used technique.ConclusionsBoth alveolar distraction osteogenesis and autogenous bone graft are effective bone regeneration techniques for the treatment of mandibular vertical bone atrophy. A level B recommendation can be established for the use of alveolar vertical distraction osteogenesis for the treatment of the mandibular vertical bone atrophy. Introduction : Les ancrages squelettiques n’ont pas été développés en tant qu’alternative aux ancrages existants habituellement utilisés. Le premier ancrage squelettique était simplement une ligature passée à travers un trou au niveau de la crête infra-zygomatique. Cette technique a été remplacée par l’utilisation de vis chirurgicales et finalement par celle de mini-vis d’ancrage temporaire (TADs), optimisées au niveau du dessin de la forme et par le choix du matériau utilisé. La tête de ces mini-vis en forme d’attache permet une utilisation comme ancrage indirect, mais ne doit pas être un moyen de contrer une perte de contrôle résultant d’un système de forces mal planifié ou d’une insuffisance de coopération. Les ancrages squelettiques doivent s’intégrer à une biomécanique adéquate, pour réaliser des traitements qui n’auraient pu l’être avant l’avènement de ces ancrages osseux. Matériel et méthode : Le but de cette étude est de tester l’hypothèse selon laquelle les ancrages squelettiques permettent de maintenir la densité osseuse, la hauteur et la largeur des procès alvéolaires dans les sites d’extraction, et de prévenir ainsi l’atrophie osseuse habituellement observée. Résultats : Pour des patients adultes avec des dentures dégénérescentes, l’utilisation des ancrages squelettiques va permettre non seulement des déplacements dentaires lorsqu’il n’y a aucune unité d’ancrage, mais aussi le redéveloppement ou la maintenance d’un os alvéolaire atrophique. Les notions élémentaires permettant l’usage optimal des ancrages squelettiques correspondent à l’utilisation d’une ligne de forces bien définie et établie pour un mouvement dentaire désiré ainsi qu’à un système de forces nécessaire et construit avec l’aide d’un ancrage squelettique soit direct ou indirect. Après une période durant laquelle les implants ostéointégrés ont été utilisés comme ancrage pour le déplacement dentaire et la conservation du capital osseux, il est maintenant admis que les mini-vis puissent servir d’ancrage pour des déplacements squelettiques évitant ainsi la mise en charge dentaire.

Background:The aim of this study was to evaluate the prognosis of minimal invasive horizontal alveolar ridge augmentation using small incision and subperiosteal tunneling technique.Methods: This study was conducted on 10 partially edentulous patients with horizontal alveolar bone deficiency in 12 areas that need horizontal alveolar ridge augmentation for future implant placement. Cone beam computer tomography was done for sites that need implant placement preoperatively. Xenogeneic bone was used for grafting deficient horizontal alveolar ridge sites using the flap technique in 6 areas and using the subperiosteal tunneling technique in another 6 areas for future implant placement. Cone beam computer tomography was done for grafted sites immediately postoperative and 6 months postoperative.Results: This study was conducted on 10 patients and included 12 implant sites. No complications related to bone grafting were found and there was an adequate clinical prognosis.Regarding the difference from pre-operatively to 6 months post-operatively, a slightly higher increase in ridge width was recorded in subperiosteal tunnel group (1.48±0.99) in comparison to flap group (0.91±0.94), with no statistically significant difference (p=0.086). Conclusions:The bone graft material retained within a pouch formed using small incision and subperiosteal tunneling technique resulted in additional increase within the width of the alveolar ridge than the flap technique.
