Depression and hypogonadism are associated with erectile dysfunction (ED). We evaluated the prevalence of both conditions in men presenting to an ED specialty clinic, and tested whether hypogonadism correlated with the presence of depressive symptoms using a validated questionnaire. From July 2001 to June 2003, 157 men referred to an ED specialty clinic prospectively filled the Center for Epidemiologic Studies Depression Scale (CES-D), the abbreviated International Index of Erectile Function (IIEF-5) and had testosterone serum levels drawn. Median age was 53 (range ¼ 21-85 years). Hypogonadism, defined as serum T (testosterone)o300 mg/dl, was present in 36% of patients. This proportion was higher in men over the median age compared to younger patients (45 and 26%, respectively, P ¼ 0.002). Overt depression symptoms, defined as a CES-DX22, were found in 24% of men. Mean age of men with overt depression was 49.9710.1 years vs 55.1715.8 years for those with CES-Do22 (P ¼ 0.02). Hypogonadal men were more likely to have overt depression scores compared to eugonadal counterparts (35 vs 18%, P ¼ 0.02). This association was statistically stronger after correcting for age in a multivariate linear model (P ¼ 0.005). The relative risk of having overt depression was 1.94 times higher in men with hypogonadal testosterone level (95% confidence interval: 1.13 to 3.7). We conclude that in an ED referral population, symptoms of hypogonadism and depression symptoms are fairly prevalent, and that overt depression symptoms are strongly associated with hypogonadism. Clinicians should consider testosterone measurements in all men with high depression symptom scores.

The baseline questionnaires included demographic and health history information. The age-adjusted prevalence of current depression by GDS (X18), of ED by IIEF-5 score (o18), and of concomitant ED and depression were 12.2%, 28.2% and 11.0%, respectively. GDS increased according to severity of ED, adjusted for age, marital status, education, smoking, alcohol, hypertension, regular exercise, total cholesterol level, fasting blood sugar, body mass index (Po0.001, by analysis of covariance). ED was strongly associated with depression symptoms after controlling for potential confounding factors using logistic regression. Compared with GDS o8, odds ratios and 95% confidence intervals for 12-17 GDS and 18 or more GDS were 3.38 (1.30-8.77) and 6.56 (2.18-19.81), respectively. ED is significantly associated with highly depressive symptoms, regardless of age, health habit or concomitant comorbidity. Our results demonstrate that multidisciplinary approaches are important for the successful treatment of ED.

The PDE5 inhibitors have recently been found to have cognitive-enhancing effects in animal models. To investigate the efficacy of repeated dosing with a PDE5 inhibitor on cognitive function and somatization in patients with erectile dysfunction, 27 patients with erectile dysfunction received udenafil (100 mg) at 3-day intervals for 2 months. The international index of erectile function-5 (IIEF-5), a cognitive battery (the Korean version of mini-mental state examination (K-MMSE), the frontal assessment battery (K-FAB), the Seoul verbal learning test) and a physical health questionnaire-15 (PHQ-15) were performed at baseline and at 2 months, following the administration of udenafil. The patients were divided on the basis of their IIEF-5 score into responders (change47) and non-responders. The mean IIEF-5 score was significantly increased after treatment (7.92±3.83 to 16.33±4.75, Po0.001). The scores of K-MMSE (27.03±1.58 to 28.07±1.57, P ¼ 0.001), K-FAB (13.65 ± 1.96 to 15.41 ± 1.85, Po0.001) and PHQ-15 (18.92 ± 4.96 to 17.63 ± 4.75, P ¼ 0.003) were significantly improved after treatment. In addition, the responders (n ¼ 16) had more improved cognitive function (r ¼ 0.603, P ¼ 0.001) and somatization (r ¼ À0.402, P ¼ 0.038) than non-responders (n ¼ 11). Repeated dosing with a PDE5 inhibitor seems to improve cognitive function and somatization, as well as erectile function in patients with erectile dysfunction.
