Efficacy and Tolerability of Latanoprost Compared to Dorzolamide Combined with Timolol in the Treatment of Patients with Elevated Intraocular Pressure: A Meta-Analysis of Randomized, Controlled Trials

Abstract: Latanoprost was associated with significantly greater efficacy in lowering diurnal mean IOP than combined dorzolamide and timolol in patients with IOP insufficiently controlled by timolol alone, and latanoprost was as effective as combined dorzolamide and timolol in patients without baseline timolol treatment. The combination of dorzolamide and timolol was less tolerated than latanoprost.

“…Of these, 39 systematic reviews (42 articles) addressed questions related to management of POAG and met our eligibility criteria (Appendix Figure 1, available at www.annals.org) (19, 21–61). Eight of the 39 were Cochrane reviews, 18 were found in the Database of Abstracts of Reviews of Effects in The Cochrane Library, and the remaining 13 were found only in PubMed or EMBASE.…”

“…Findings from a systematic review were classified as at high risk of bias if it contained a non-comprehensive literature search, did not assess the methodological quality of included studies, used inappropriate statistical methods for meta-analysis, or presented conclusions inconsistent with the review findings (for example, if the review finding was based on 1 statistically significant meta-analysis out of 26 meta-analyses conducted [19]). Findings were classified as at low risk of bias if none of these deficiencies was observed and as at unclear risk of bias if insufficient details were provided to permit judgment on 1 or more of the 4 deficiencies (Table 1) (20).…”

“…20 Cheng et al reported that the combination of dorzolamide and timolol was less tolerated than latanoprost. 30 Renieri et al noted that preservative-free DTFC was well tolerated and improved the local tolerability in glaucoma patients. 31 Shedden et al compared the efficacy and tolerability of preservative-free and preservative-containing formulations of DTFC in patients with elevated IOP and noted that both combinations were equivalent in efficacy for change in trough and peak IOP, and generally had similar tolerability.…”

“…Most other ocular adverse events (including visual field defects, cataracts, ocular hyperemia) were mild to moderate in intensity, and occurred independent of presence of increased iris pigmentation. 215 Compared with other topical ocular hypotensive medications, higher discontinuation rates were found for bimatoprost (5.3%) than for timolol (1.7%)123 and dorzolamide/timolol combination than for latanoprost, 186 although similar for bimatoprost (3.3%) and FCDT (3.4%). 132 Compared with latanoprost, ocular adverse effects were similar to dorzolamide.…”
