Doripenem is a carbapenem antibiotic with broad-spectrum coverage of gram-positive and gram-negative bacteria, including Pseudomonas aeruginosa, and is considered to be as effective as meropenem. The in vivo activity of doripenem was thus compared with that of meropenem in a chronic lower respiratory P. aeruginosa infection mouse model. The number of viable bacteria in the lungs after treatment with doripenem, meropenem, and saline mice was 2.01 ± 0.69, 2.03 ± 0.48 and 3.90 ± 1.40 log 10 CFU/lung, respectively. The number of viable bacteria in the lungs of mice treated with doripenem and meropenem was significantly lower than that in lungs of controls.Histopathological examination of lung specimens from the control group revealed promotion of the inflammatory response in chronic bronchial infection. However, the groups treated with doripenem and meropenem showed weaker inflammatory responses. These results suggest that doripenem treatment is effective against chronic airway infection with P. aeruginosa.

ÖzetAmaç: Acinetobacter türleri son yıllarda hastanemiz yoğun bakım hastaların-da en sık rastlanan ilk iki etken arasında yer almaktadır. Ülkemizde kullanıma yeni sunulan doripenem, bakteri hücre duvarı sentezini inhibe ederek etkinlik gösteren, karbapenem grubu geniş spektrumlu bir antibiyotiktir. Çalışmalar-da özellikle ventilatör ilişkili pnömoni etkeni olarak izole edilen Acinetobacter baumannii suşlarında doripenemin duyarlılıkları araştırılmıştır. Çalışmamız-da nozokomiyal infeksiyonlarda etken olarak izole edilen Acinetobacter baumannii suşlarının doripenem duyarlılıklarının saptanmasında disk diffüzyon ve E-test yöntemlerinin karşılaştırmayı amaçladık. Gereç ve Yöntem: Akdeniz Üniversitesi Merkez Laboratuvarı'na 1 Ocak-31 Aralık 2009 tarihleri arasında yoğun bakımlardan gelen çeşitli klinik örneklerden izole edilen toplam 94 Acinetobacter baumannii suşunun disk diffüzyon ve Etest yöntemleri ile doripenem duyarlılıkları belirlenmiştir. European Committee on Antimicrobial Susceptibility Testing (EUCAST) tarafından belirlenen minimal inhibitör konsantrasyonları (MİK); duyarlı ≤1 μg/ml, orta duyarlı 2-4 μg/ml ve dirençli >4 μg/ml olarak ve 10 μg'lık disklerle inhibisyon zonu çapları, duyarlı <15mm, direnç-li ≥21mm olarak kullanılmıştır. Bulgular: Disk diffüzyon yöntemiyle; 10 izolat duyarlı, 17 izolat orta duyarlı, 67 izolat ise dirençli saptanırken, E-test yönte-miyle; 12 izolat duyarlı, 12 izolat orta duyarlı, 70 izolat ise dirençli saptanmış-tır. E-test yöntemi ile disk diffüzyon yöntemi arasındaki kategorik uyum (orta duyarlı izolatlar dirençli kabul edilerek), %88.3 (83/94) olarak hesaplanmış-tır. Çalışmamızda büyük hata, ve küçük hata oranları sırasıyla %1.2 (1/82) ve %18.1 (17/94) olarak saptanmış, çok büyük hata saptanmamıştır. Tartışma: Sonuç olarak rutin laboratuvar pratiğinde Acinetobacter türlerine karşı doripenem duyarlılıkları saptanırken disk diffüzyon ile E-test yöntemlerinin yük-sek düzeyde uyum gösterdiği tespit edilmiştir. Background: Although guidelines and reviews have systematically evaluated diagnosis and surgical management of acute diverticulitis, they have focused only minimally on antibiotic selection for the treatment of this disease. We undertook a review of the literature to assess more clearly the use of specific antimicrobial agents in the treatment of patients with acute diverticulitis of the colon. Methods: A MEDLINE search was conducted to identify original research, review papers, and guidelines on the use of antimicrobial agents for the treatment of acute diverticulitis. Results:The general recommendation to use antibiotics with activity against common gram-negative and anaerobic pathogens has remained consistent. A number of single agents and combination regimens provide such activity. However, there is little evidence on which to base selection of specific antimicrobial regimens, and no regimen has demonstrated superiority. In general, episodes of diverticulitis severe enough to warrant hospitalization should be managed initially with intravenous antibiotics. Oral therapy can be used for outpatient treatment or when the patient's condition improves. There is a paucity of data regarding the optimal duration of antimicrobial therapy. Conclusions: Careful clinical studies are needed to evaluate better the antibiotic regimens for the treatment of acute diverticulitis. Until such studies are conducted, we are forced to rely on tradition, in vitro analyses, pharmacokinetic profiling, and indirect evidence from studies of complicated intra-abdominal infections to determine appropriate therapy for this disease.
