Background The mortality and morbidity rate of diabetes patients is increasing worldwide which requires an ideal treatment to prevent the disease worsening. Traditional medicine is gaining more attention in diabetes due to its efficacy and safety. We, therefore performed a systematic review study of clinical trials to assess the comparative effect of polyherbal formulations in type 2 D iabetes mellitus . Objectives To find the effectiveness of polyherbal formulations in blood sugar and lipid level for type 2 D iabetes mellitus . Material and methods PubMed, Scopus and CINAHL databases for clinical trials investigating the effect of polyherbal formulations in Type 2 D iabetes mellitus patients were searched. Meta-analysis of eligible trials was conducted employing Revman 5.2 software. Results Fourteen randomized controlled trials were found eligible for meta-analysis. Meta-analysis of findings showed a significant effect of polyherbal formulations on blood sugar level compared to control group. The estimated standard mean changes at 95% confidence interval, following polyherbal formulations treatment were −0.59, (−0.91 to – 0.27) mg/dL; for fasting blood sugar(p < 0.001), −0.69, (−1.18 to −0.21) mg/dL; for postprandial blood sugar (p = 0.005) and −0.46, (−0.88 to −0.04) gm%; for glycated haemoglobin (p = 0.03). The reduction in postprandial sugar and glycated haemoglobin was statistically significant with polyherbal formulations compared to metformin treatment but not for fasting sugar. Similarly in lipid profile the reduction for total cholesterol and triglycerides was statistically significant with polyherbal formulations compared to control group but was not significant for HDL and LDL whereas in other group of polyherbal formulations and metformin only HDL was favouring polyherbal formulations. Conclusion Polyherbal formulations occurred to be effective in lowering blood sugar level in Type 2 diabetes but their further efficacy in managing diabetes needs to be validated. Therefore, a qualitative, long term, randomized placebo-controlled trials of adequate sample size are necessary to determine the efficacy of polyherbal formulation in managing diabetes.

Ayurveda, a traditional medicinal system originating in India, is founded on the principle of averting needless misery and encouraging a long, healthy life. Its method entails restoring equilibrium by addressing the underlying cause of illnesses with natural components, while also advocating for a balanced lifestyle to stop imbalances from happening again. Herbal medicines, with a rich historical presence in various cultures such as Indian, Greek, Egyptian, and Chinese have all been used medicinally. The World Health Organization estimates that 80% of people on the planet mostly get their medical care from traditional medicines. Known as a significant hub for biodiversity, India is home to around 45, 000 plant species, of which 15, 000 are known to have medicinal properties. Communities in India utilize 7, 000-7, 500 plants for treating various ailments. In Ayurveda, both individual and combinations of herbs, known as polyherbal formulations, are employed for treatment. The 'Sarangdhar Samhita' in Ayurvedic literature underscores the importance of polyherbalism in boosting therapeutic effectiveness. Recognizing that the It's possible that specific plants' active phytochemical components are inadequate.for desired therapeutic effects, Ayurveda emphasizes the synergistic benefits achieved by combining multiple herbs in specific ratios. This approach not only enhances therapeutic effects but also minimizes potential toxicity. Polyherbal formulations, as highlighted in Ayurvedic literature, leverage the concept of 'panchamahabhutas, ' emphasizing the balance of five elemental forces in the body. Furthermore, Ayurveda recognizes the holistic nature of health, addressing physical, mental, and spiritual well-being. The integration of polyherbalism aligns with Ayurvedic principles of personalized medicine, where formulations are tailored to an individual's unique constitution or dosha. This review delves into the critical role of polyherbalism in Ayurveda, exploring its clinical significance and the intricate balance it seeks to achieve in promoting comprehensive well-being. Key terms such as Ayurveda, panchamahabhutas, and polyherbal formulation are pivotal in understanding the depth and breadth of this traditional medicinal system.

Impact of Antioxidants on Antidepressant ActivityIn Ayurveda, single or multiple herbs are used as medication for various ailments. Depression is often manifesting with various symptoms at the behavioral, psychological and physiological levels. Therefore, the investigation for therapeutic alternative is important. Oxidative stress has shown important biochemical aspects in the depression. This study evaluated effect of natural antioxidant on antidepressant activity of polyherbal formulation on the performance of male mice. Mice were given orally polyherbal formulation without antioxidant and with antioxidant daily for 7 d and then subjected to forced swim test and tail suspension test. After 1 w treatment, both polyherbal formulation significantly reduced immobility time in forced swim test and tail suspension test compared with vehicle treated control group. The immobility time in tail suspension test of polyherbal formulation without antioxidant and with antioxidant was found to be 151.17±4.46 s and 116.33±8.84 s respectively. The immobility time in tail suspension test of polyherbal formulation without antioxidant and with antioxidant was found to be 137.17±5.93 s, 113.50±5.40 s respectively. These results indicate that the antidepressant when given along with antioxidant in mice it gives significant antidepressant effect. The experimental results suggest that the intake of antioxidant may help in reducing the symptoms of depression, via supplementation of antioxidant.
