Background Sinupret drops is commonly used as agent dissolving nasal secretions. However, there is no specific regime of Sinupret drops for pediatric patients in China. Therefore, we investigate its current medication in Chinese pediatric patients to provide a recommendation of safe drug use.Methods We collect 6,893 outpatient and emergency pediatric prescriptions of Sinupret drops (age ≤ 15) and focused on the analysis of practical drug dosage and potential risks.Results One-half of the adult dosage was mainly chosen for out-patient children, while the full adult dosage was much more used for pediatric patients in emergency. Its usage in low-age pediatric patients requires further evaluation and surveillance. The pharmaceutical reaction of ethanol in Sinupret drops with some cephalosporins during drug taking will lead to unexpected drug reactions.Conclusion To prevent potential side effects, treatment with Sinupret drops as an adjunct for pediatric patients should draw more attention from their doctors and pharmacists. The age and weight of pediatric patients need to be considered as important parameters for drug administration of Sinupret drops. It is important to strengthen the education of medication, and much attention should be paid to the interval of drug administration.

The clinical pattern of rhinosinusitis is based on key symptoms, including nasal obstruction/congestion, nasal secretion with postnasal discharge, facial pain with the sense of pressure, as well as impaired sense of smell. Acute rhinosinusitis usually becomes as a viral infection due to the impaired paranasal sinus aeration and drainage and this viral infection can be followed by bacterial superinfection. Chronic rhinosinusitis is an inflammatory disease of the sinonasal mucosa with symptoms persisting for more than twelve weeks. The aim of this paper is to summarize and update the relevant literature related to the use of herbal medicinal product Myrtol® standardized in the therapy of acute and chronic rhinosinusitis. According to the results of previous investigations, Myrtol® standardized is found to have a strong secretolytic, secretomotoric, antiinflammatory, antioxidative and antimicrobial effects. A review of the literature was carried out on randomized studies related to the use of this preparation in therapy of these forms of upper airway inflammations. We, also discussed the mechanisms of action of this herbal drug on diseased nasal and paranasal sinuses mucosa.

Dysosmia is a frequent symptom in acute rhinosinusitis. Post-infectious dysosmia has a large share in the structure of olfactory disorders. The most common is postviral dysosmia. The sense of smell in acute rhinosinusitis can change both in the conductive and sensorineural types, but most often it is of a mixed nature, has an acute beginning and a favorable prognosis. There is also a transformation of the olfactory epithelium into a multilayer flat or respiratory epithelium. The pathogenesis of acute rhinosinusitis is based on an increase in edema of the mucous membrane of the upper respiratory tract and, as a result, the block of the natural sinuses of the paranasal sinuses, which leads to a violation of their ventilation and stagnation of secretions in the sinus cavity. For the treatment of acute rhinosinusitis, antibacterial, vasoconstrictive drugs, elimination and irrigation therapy are used.it is important to restore mucociliary transport, and therefore it is necessary to prescribe mucoactive and secretolytic drugs. A special role among these medicines is played by herbal preparations, among which one of the most effective and well-studied is a combined herbal preparation that includes Gentiana lutea, Primula veris, Rumex acetosa, Sambucus nigra, Verbena officinalis. Due to the anti-inflammatory and secretolytic effects, treatment regimens that include this drug provide high efficiency and optimal treatment times for patients with acute rhinosinusitis. Studies have shown that this drug has shown inhibitory activity in vitro against respiratory viruses, including respiratory syncytial virus, adenovirus and parainfluenza. In addition, it has been proven that when using this drug, a pronounced anti-inflammatory effect develops in the form of a decrease in prostaglandin E2 levels, cyclooxygenase-2 expression, tumor necrosis factor, and especially interleukin. In addition, the inclusion of this drug in the complex therapy of acute rhinosinusitis contributes to the restoration of olfactory function in the form of reducing the threshold of smell, increasing the ability to distinguish and identify odors. Data from the studies presented in the article demonstrate high efficacy and good tolerance of this drug.
