Efficacy and safety of antiviral treatment on blocking the mother‐to‐child transmission of hepatitis B virus: A meta‐analysis

Abstract: Summary Nucleo(t)side analogues (NAs) have been administered as adjunctive therapy to interrupt the mother‐to‐child transmission (MTCT) of hepatitis B virus (HBV). The efficacy and safety of this method remain controversial. A meta‐analysis was conducted to evaluate the efficacy and safety of NAs treatment during pregnancy. The differences among different agents and initiation trimesters were analysed. A total of 9228 mother‐infant pairs in 59 studies (32 RCTs and 27 non‐RCTs) were included in this meta‐analys… Show more

“…A meta‐anlysis 19 comparing lamivudine, telbivudine and tenofovir did not show any statistically significant difference in the three antivirals. Telbivudine and tenofovir are currently rated pregnancy category B, and lamivudine pregnancy category C, by the US Food and Drug Administration based primarily on animal data, with no clear evidence of harm in sparse human data.…”

“…A meta analysis has shown a significant reduction in MTCT of HBsAg with nucleos(t)ide analogues with a RR = 0.51, 95% CI 0.45‐0.57 19 . This is now recommended in all guidelines.…”

“…Another meta-analysis showed all recommended agents have a great effect on preventing vertical transmission of HBV and second trimester was the better timing of initiating AVT in the subgroup analysis of HBV DNA/HBsAg positivity [48]. Major of them were based on the pair-wised meta-analysis [6,48]. Therefore, different from existing studies, our network analysis has some following remarkable characteristics.…”

“…Brown et al recommended the use of these agents in female who are HBeAg positive and high level of HBV DNA in the third trimester to prevent MTCT [6]. “…Although TDF is detected in breast milk, it has low oral bioavailability, and infants are exposed to minimal oral concentrations (<0.03% of recommended neonatal dose) . In nonpregnant patients with CHB, long‐term TDF therapy is linked to metabolic bone disease and renal dysfunction, but to date no significant maternal safety concerns have been reported with NA therapy during pregnancy . A new formulation of TDF (tenofovir alafenamide fumarate [TAF]) was recently approved for treatment of CHB and human immunodeficiency virus (HIV) infection.…”
