Effects of Voglibose on Glycemic Excursions, Insulin Secretion, and Insulin Sensitivity in Non-lnsulin-ireated NIDDM Patients

Abstract: The results suggest that voglibose lowers the daily glycemic excursions and inhibits overwork of the pancreatic beta-cells but has little effect on insulin sensitivity in NIDDM patients.

“…When glycemic control is good, 1,5-AG levels rise, 8,20 and when glycemic control is poor, 1,5-AG levels fall. 21 Serum 1,5-AG levels were measured with an enzymatic method using 1,5-AG Auto Liquid reagent (Nippon Kayaku Co., Ltd, Tokyo) and an automatic clinical analyzer (model 7150; Hitachi, Tokyo).…”

“…4 An automated assay for serum 1,5-anhydroglucitol (1,5-AG), as a new biochemical marker, has been first used in Japan since 1991, and a similar assay (GlycoMark 1,5-AG test; GlycoMark, Inc., New York, NY) has been approved in the United States. 5,6 The serum 1,5-AG level has been shown to generally reflect postprandial hyperglycemia 7 but not fasting hyperglycemia 8 and to be strongly and negatively correlated with the plasma glucose level at 120 min of the oral glucose tolerance test in subjects with IGT. 7 Postprandial glycemic levels have been difficult to monitor, but 1,5-AG levels have proven to be beneficial for this purpose.…”

“…Developed in Japan as a new marker for glycaemia [7][8][9][10][11][12][13][14], the measurement of 1,5-anhydroglucitol (1,5-AG) in serum can detect slight changes in the glycaemic state, and therefore appears well suited to monitoring glucose homoeostasis in patients with near-normoglycaemia [13,15,16] or postprandial hyperglycaemia [13,17,18]. Within its epidemiologically defined normal range, 1,5-AG has an individual normal value for each patient, and falls rapidly from this norm (by as much as hundreds of milligrams of glucose per day) if it is being excreted continuously in the urine [19].…”

“…In addition, validamycin can be used as the source for chemical synthesis of voglibos, an important antidiabetic drug (Matsumoto et al, 1998). Considerable efforts have been devoted to understand the validamycin biosynthesis and fermentation regulation.…”
