The aim of this study was to detect possible risk factors for UC and UTI following pediatric renal Tx and effect of these complications on outcome. One hundred and eight children who underwent living donor Tx between 2009 and 2015 were retrospectively included. Extraperitoneal approach was used with stented tunneled extravesical procedure. Mean recipient age was 9.89 ± 3.46 years while mean weight was 25.22 ± 10.43 kg. Seventy-three (67.6%) recipients were boys while 92 (85.2%) were related to donors. Urological causes of ESRD were present in 33 (30.6%) recipients (14 [13%] posterior urethral valve, 16 [14.8%] VUR, and 3 [2.8%] neurogenic bladder). Augmentation ileocystoplasty was performed in 9 (8.3%) patients. Mean follow-up was 39.3 ± 17.33 months. UC were detected in 10 (9.3%) children (leakage 4 [3.7%], obstruction 3 [2.8%], and VUR 3 [2.8%]) while UTIs were reported in 40 (37%) children. After logistic regression analysis, UC were significantly higher in children with cystoplasty (44.4% vs 6.1%; P = .001). UTIs were significantly higher in girls (51.4% vs 30.1%; P = .001) and in children with urological causes of ESRD (51.5% vs 30.7%; P = .049). UC and UTI were not significantly associated with increased graft loss or mortality. UC were significantly higher in children with cystoplasty while UTIs were significantly higher in girls and children with urological causes of ESRD. Presence of UC did not affect the rate of graft loss or mortality due to its early detection and proper management.

Renal Transplant Outcome in Children with an Augmented Bladder

Objective: Studies evaluating renal transplant (RT) outcome in children who underwent an augmentation cystoplasty (AC) are contradictory and the current knowledge is based on studies with a limited number of patients. The aim of this study is to compare RT outcome between children who underwent AC and those without augmentation.Patients and methods: A total of 20p who underwent an AC prior to the RT (12 with ureter and 8 with intestine) were enrolled in the study and were compared to a control group of 24p without AC, transplanted in the same time period (1991–2011). Data including; age at transplant, allograft source, urological complications, urinary tract infections (UTI) incidence, the presence of VUR, and patient and graft survival were compared between the groups.Results: Mean age at RT and mean follow-up were 9.7 vs. 7.9 years and 6.9 vs. 7.9 years in the AC group and control group, respectively (NS). The graft originated in living donors for 60% of AC patients and 41.6% of the control RT patients. The rate of UTI were 0.01 UTI/patient/year and 0.004 UTI/patient/year in the augmented group and controls, respectively (p = 0.0001). In the AC group of 14p with UTIs, 10 (71%) had VUR and 5p out of 8 (62.5%) in the control group had VUR. In the AC group, of the 7p with ≥3 UTIs, 3 (43%) were non-compliant with CIC and the incidence of UTIs was not related with the type of AC or if the patient did CIC through a Mitrofanoff conduit or through the urethra. Graft function at the end of study was 92.9 ± 36.85 ml/min/m2 in the AC group and 88.17 ± 28.2 ml/min/m2 in the control group (NS). Graft survival at 10 years was also similar 88% in the AC group and 84.8% in controls. In the AC group 3p lost their grafts and 5 in the control group with respective mean follow-up of 10.6 ± 4.3 and 7.1 + 4.7 years.Conclusion: There are no significant differences in the RT outcome between children transplanted with AC or without. However, recurrent UTIs are more frequent in the former group and these UTIs are related with non-compliance with CIC or the presence of VUR but, even so, UTIs will not lead to impaired graft function in most of the patients.

Özet Doğumsal ürolojik sorunlara bağlı son dönem böbrek yetmezliği (SDBY) gelişen çocukların tedavisi diğer nedenle- GirişDoğumsal ürolojik sorunlara bağlı son dönem böb-rek yetmezliği (SDBY) gelişen çocukların tedavisi diğer nedenlere bağlı SDBY gelişen hastalara göre daha problemli ve zor olmaktadır (3) . Bu zorlukların en başında uygun kapasite, kompliyans ve işlevi olmayan çoğu nörojenik kökenli mesaneye sahip hasta grubuna, en seçkin renal replasman tedavisi (RRT) olan böbrek naklinin uygulanma süreci ve kararı gelmektedir. Günümüzde cerrahi ve medikal tedavilerde gelişmeler sayesinde bu hastalarda mesane büyütücü girişimler (augmentation) yapılarak uygun mesane hacmi ve optimal basınç sağlanabilmektedir (4) .İlk kez Stephenson ve ark.(7) tarafından 1984 yılında yeniden şekillendirilmiş mesane sonrasında yapılan böbrek naklinden bu yana yaklaşık 30 yıllık bir süre geçmesine karşın literatürde bu özel hasta grubu ile ilgili bilgi ve deneyim sınırlıdır. Bu çalışmada posterior üreteral valv (PUV), valv mesanesi, bilateral ağır vezikoüreteral reflü (VUR) tanılarıyla izlediğimiz ve geç tanı nedeniyle SDBY gelişmiş bir çocuk hastada mesane büyütme ameliyatı (augmentation) uygulamasından sonra yapılan böbrek nakli ile ilgili deneyimimiz aktarılmıştır.

Abstract: Our study demonstrated that patients with transplanted kidney and augmented bladder had more asymptomatic bacteriuria and urinary tract infections than those without bladder augmentation. However, the rate of graft survival was similar between the groups.
