Effects of the Selective Serotonin Reuptake Inhibitor Fluoxetine on Counterregulatory Responses to Hypoglycemia in Individuals With Type 1 Diabetes

Abstract: OBJECTIVE-Previous work has demonstrated that chronic administration of the serotonin reuptake inhibitor (SSRI) fluoxetine augments counterregulatory responses to hypoglycemia in healthy humans. However, virtually no information exists regarding the effects of fluoxetine on integrated physiological counterregulatory responses during hypoglycemia in type 1 diabetes. Therefore, the specific aim of this study was to test the hypothesis that 6-week use of the SSRI fluoxetine would amplify autonomic nervous system … Show more

“…This postulation fits well with the ability of drugs that interfere rather than enhance 5-HT uptake (like doxepin, sertraline, paroxetine or fluvoxamine) to counteract hyper-insulinism + hypoglycemia syndrome [16,53].…”

“…All enhance counterregulatory responses to falling glucose concentrations. 66 SSRIs increase the counterregulatory responses to hypoglycemia in humans 67 and rats. 68 A randomized clinical trial of an SSRI for the prevention of iatrogenic hypoglycemia may be warranted.…”

“…Five potential treatments for reversing glucose counterregulatory defects 66 -selective serotonin-reuptake inhibitors (SSRIs), 67,68 adrenergic antagonists, 69 an opiate-receptor antagonist, 70,71 fructose, 72 and a selective ATP-sensitive potassium (K ATP )-channel agonist 73 -are of particular interest. All enhance counterregulatory responses to falling glucose concentrations.…”

“…We (37) and others (3,46) have shown that serotonergic signaling promotes the autonomic counterregulatory responses to acute IIH. Sanders et al (46) demonstrated that a 21-day pretreatment with sertraline prevented attenuation of the adrenomedullary response by RH.…”

“…Although 21 days of pretreatment with sertraline (7.5 mg·kg Ϫ1 ·day Ϫ1 sc) had previously been shown to amplify the counterregulatory response to acute hypoglycemia and prevent blunting of the CRR after RH (46), treatment with fluoxetine, another selective serotonin reuptake inhibitor, failed to amplify the behavioral symptoms associated with a single bout of IIH in humans (3). Therefore, we hypothesized that sertraline pretreatment would not prevent the IIH prevention of CPP as seen after recurrent bouts of IIH (Fig.…”
