BackgroundDry needling (DN) and percutaneous electrical nerve stimulation (PENS) are widely used techniques in the treatment of myofascial pain.ObjectiveTo investigate the immediate and short-term effects of the combination of DN and PENS compared to DN alone on the upper trapezius muscle.MethodThis is a 72-hour follow-up single-blinded randomized controlled trial. Sixty-two volunteer patients with chronic myofascial neck pain with active Myofascial Trigger Points (MTrPs) in the upper trapezius muscle were recruited. Randomization was performed, and 31 patients received DN treatment (DN group) and 31 received DN and PENS (DN+PENS group). The primary outcomes were neck disability index (NDI) and visual analog scale for pain for both post-needling soreness (PNS) and neck pain intensity (NPI). Pressure pain threshold (PPT) and cervical range of motion (CROM) were the secondary outcomes. ResultsWe detected between-group differences in NPI and PNS in favor of the DN+PENS group immediately after treatment. No between-group differences in NDI were observed.ConclusionPENS application after dry needling treatment is more effective than dry needling alone for decreasing soreness in the short term and improving neck pain intensity immediately in patients with myofascial chronic neck pain.

Somatic awareness (SA) refers to heightened sensitivity to a variety of physical sensations and symptoms. Few attempts have been made to dissociate the relationship of SA and affective symptoms with pain outcomes. We used a validated measure of mood and anxiety symptoms that includes questions related to SA to predict the number of tender points found on physical examination in a large cross-sectional community sample (the Midlife in the United States [MIDUS] Biomarker study). General distress, positive affect, and SA, which were all significantly associated with tender point number in bivariate analyses, were used as predictors of the number of tender points in a multivariate negative binomial regression model. In this model a greater number of tender points was associated with higher levels of SA (p = .02) but not general distress (p = .13) or positive affect (p = .50). Follow-up mediation analyses indicated that the relationship between general distress and tender points was partially mediated by levels of SA. Our primary finding is that SA is strongly related to the number of tender points in a community sample. Mechanisms linking SA to the spatial distribution of pain sensitivity should be investigated further. Perspective This article presents an analysis of three overlapping psychological constructs and their relationship to widespread pain sensitivity on palpation. The findings suggest that somatic awareness is most strongly related to the spatial distribution of pain sensitivity and that further assessing it may improve our understanding of the relationship between psychological factors and pain.

resUMenEl dolor es un fenómeno complejo y multifactorial que depende de la interacción de factores biopsicosociales. Entre el 15-25% de los adultos sufren dolor cró-nico en algún momento de su vida. A nivel cervical el dolor crónico es considerado un problema de salud pú-blica y afectó a un 9,6% de hombres y a un 21,9% de mujeres según la última Encuesta Nacional de Salud 2011-12. Un porcentaje muy elevado de consultas por dolor muscular en atención primaria resulta ser por síndrome de dolor miofascial. Su existencia implica la presencia de puntos gatillo miofasciales que pueden encontrarse latentes o activos en toda la población. El objetivo de esta revisión es exponer los métodos terapéuticos más utilizados aplicados por el fisioterapeuta en el tratamiento de este síndrome a nivel cervical. Algunas de las técnicas más utilizadas que pueden resultar útiles a corto o medio plazo son: la compresión isquémica y/o liberación por barrera, y la punción seca. Asimismo, para tratar este síndrome también son utilizadas distintas combinaciones de modalidades terapéuticas teniendo en cuenta otros aspectos como el educativo.Palabras clave. Cervicalgia.
