Objective: Many tinnitus patients complain about difficulties regarding speech comprehension. In spite of the high clinical relevance little is known about underlying mechanisms and predisposing factors. Here, we performed an exploratory investigation in a large sample of tinnitus patients to (1) estimate the prevalence of speech comprehension difficulties among tinnitus patients, to (2) compare subjective reports of speech comprehension difficulties with behavioral measurements in a standardized speech comprehension test and to (3) explore underlying mechanisms by analyzing the relationship between speech comprehension difficulties and peripheral hearing function (pure tone audiogram), as well as with co-morbid hyperacusis as a central auditory processing disorder.Subjects and Methods: Speech comprehension was assessed in 361 tinnitus patients presenting between 07/2012 and 08/2014 at the Interdisciplinary Tinnitus Clinic at the University of Regensburg. The assessment included standard audiological assessments (pure tone audiometry, tinnitus pitch, and loudness matching), the Goettingen sentence test (in quiet) for speech audiometric evaluation, two questions about hyperacusis, and two questions about speech comprehension in quiet and noisy environments (“How would you rate your ability to understand speech?”; “How would you rate your ability to follow a conversation when multiple people are speaking simultaneously?”).Results: Subjectively-reported speech comprehension deficits are frequent among tinnitus patients, especially in noisy environments (cocktail party situation). 74.2% of all investigated patients showed disturbed speech comprehension (indicated by values above 21.5 dB SPL in the Goettingen sentence test). Subjective speech comprehension complaints (both for general and in noisy environment) were correlated with hearing level and with audiologically-assessed speech comprehension ability. In contrast, co-morbid hyperacusis was only correlated with speech comprehension difficulties in noisy environments, but not with speech comprehension difficulties in general.Conclusion: Speech comprehension deficits are frequent among tinnitus patients. Whereas speech comprehension deficits in quiet environments are primarily due to peripheral hearing loss, speech comprehension deficits in noisy environments are related to both peripheral hearing loss and dysfunctional central auditory processing. Disturbed speech comprehension in noisy environments might be modulated by a central inhibitory deficit. In addition, attentional and cognitive aspects may play a role.

Background Tinnitus, the perception of a sound in absence of an external auditory source, can significantly impact ones' quality of life. As tinnitus is often associated with hearing loss, hearing aids have been proposed for tinnitus relief in literature for more than 70 years. While there is a long history of clinical work and research on this topic, there is a need for recent literature to be reviewed and guide decision making in tinnitus management. Objective The aim of this scoping review is to provide an update of the available evidence on hearing aids for tinnitus, focusing on the effect of sound amplification, to draw conclusions for clinical practice and identify gaps in the field. A consultation exercise was included to discuss current issues that practitioners and carers themselves face but remain under-researched. Design This scoping review was conducted based on the six-stage framework of Arksey et al. (2005). Studies were included if they investigated hearing aids for tinnitus and were published after 2011. Databases of PubMed and Scopus were explored on the 16th of November 2020. The search was limited to English manuscripts. A total of 28 primary research studies were selected. Results Positive results of hearing aids for tinnitus relief were shown by 68 % of the studies, whereas 14 % demonstrated no change in tinnitus perception. As the quality of the evidence across studies was variable, no consensus can be reached regarding the use of hearing aids as a treatment for tinnitus. Nevertheless, recent studies were more likely to focus on optimizing the effect of hearing aids and better predicting which tinnitus patients benefit from hearing aids. The experts stated that the findings were in agreement with their view on the scientific evidence and they emphasized the importance of reaching consensus. Conclusions The majority of the studies supported the use of hearing aids for tinnitus relief. Hence, there was some scientific support for it, but the quality of evidence was questioned. Stronger methodology in future studies is needed to reach consensus and support clinical guidelines development.

Introdução: O zumbido juntamente com a perda auditiva, é uma percepção acústica anormal que pode levar a uma desorganização no Sistema Nervoso Auditivo Central. Por isso sujeitos portadores são suscetíveis ao Transtorno do Processamento Auditivo Central. O Treinamento Auditivo, baseado nos conceitos de neuroplasticidade pode auxiliar na redução do incômodo com o zumbido, por objetivar sincronia na via auditiva. Objetivo: Estimar os efeitos do Treinamento Auditivo Acusticamente Controlado Computadorizado na redução do incômodo com o zumbido e nas alterações das habilidades auditivas em idosos com perda auditiva usuários de próteses auditivas. Material e Método: Estudo transversal quantitativo e qualitativo, no qual foram reabilitados cinco idosos com zumbido, alteração em pelo menos uma habilidade auditiva e perda auditiva. Foram realizadas avaliações pré e pós intervenção, sendo elas: anamnese e avaliação audiológica básica, aplicação do questionário Tinnitus Handicap Inventory, testes comportamentais do processamento auditivo e avaliação eletrofisiológica. Realizou-se o tratamento com o software Escuta Ativa, em 16 sessões de em média 30 minutos cada. Resultados: Houve diferença estatisticamente significante em relação aos valores pré e pós tratamento no questionário Tinnitus Handicap Inventory e nos testes comportamentais. Não houve mudanças eletrofisiológicas pré e pós tratamento. Conclusão: Foi possível avaliar os efeitos do Treinamento Auditivo Acusticamente Controlado Computadorizado na redução do incômodo com o zumbido, por meio do questionário escolhido aplicado pré e pós intervenção. Além disso, algumas habilidades auditivas comportamentais também melhoraram com a referida intervenção. Apenas não foi possível observar mudanças eletrofisiológicas no Potencial Evocado Auditivo Longa Latência.
