Smoking is a well established risk factor for coronary artery disease (CAD). Despite this, there have been no previous studies investigating the effects of smoking on blood gene expression in CAD patients. This single-centre cross-sectional study was designed with clearly defined inclusion criteria to address this gap. We conducted a high-throughput approach using next generation sequencing analysis with a single-end sequencing protocol and a read length of 75-cycles. Sixty-one patients with a median age of 67 years (range: 28–88 years) were recruited, and only 44 subjects were included for further analyses. Our investigation revealed 120 differentially expressed genes (DEGs) between smokers and nonsmokers, with a fold change (FC) of ≥1.5 and a p-value < 0.05. Among these DEGs, 15 were upregulated and 105 were downregulated. Notably, when applying a more stringent adjusted FC ≥ 2.0, 31 DEGs (5 upregulated, annotated to immune response pathways, and 26 downregulated, involving oxygen and haem binding or activity, with FDR ≤ 0.03) remained statistically significant at an alpha level of <0.05. Our results illuminate the molecular mechanisms underlying CAD, fortifying existing epidemiological evidence. Of particular interest is the unexplored overexpression of RCAN3, TRAV4, and JCHAIN genes, which may hold promising implications for the involvement of these genes in CAD among smokers.

Latar belakang: wanita memiliki risiko untuk memiliki kadar hemoglobin yang rendah, namun bagi perokok memiliki risiko kadar hemoglobin yang tinggi. Pengaruh merokok pada kualitas kadar hemoglobin pada tubuh dapat mengindikasikan suatu gangguan kesehatan. Tujuan: Untuk mengetahui kadar hemoglobin pada wanita perokok dan bukan perokok. Metode: Penelitian ini merupakan penelitian dengan desain cross sectional dengan analisis data sekunder IFLS 5 melibatkan matching sampel 127 wanita perokok dan 127 wanita bukan perokok menggunakan Propensity Score Matching (PSM). Hasil: Pada penelitian ini didapatkan kadar hemoglobin wanita perokok cenderung lebih tinggi jika dibandingkan dengan wanita bukan perokok bahkan setelah dilakukan matching, dimana secara kategorik status nilai Hb didapatkan 5 wanita termasuk memiliki kategori kadar Hb yang tinggi (>15 gr/dl). Secara analisis numerik kadar hemoglobin pada wanita perokok adalah 13.1 (7.3-17) sedangkan kadar hemoglobin wanita bukan perokok adalah 12.8 (7.1-15.3). Pada pengelompokkan wanita perokok berdasarkan lama Riwayat merokok didapatkan nilai kadar Hb tertinggi ditemukan pada kelompok wanita perokok dengan Riwayat merokok selama 3 tahun lebih. Hasil uji korelasi didapatkan bahwa terdapat korelasi positif antara nilai IMT dengan kadar Hb. Kesimpulan: Setelah dilakukan matching sampel antara wanita perokok dan bukan merokok kadar Hb pada kelompok perokok wanita masih terlihat cenderung lebih tinggi dibandingkan kelompok wanita bukan perokok. Selain itu kualitas kadar Hb pada seorang perokok juga harus mendapat perhatian lebih karena merokok dapat meningkatkan kadar karbon monoksida pada darah. Kata kunci : IFLS, kadar hemoglobin, matching sampel, wanita perokok.

In Indonesia, cigarette consumption is quite prevalent and has been become more common over time. According to Riskesdas (Baseline Health Research from Agency of Health Research and Development) study in 2018, the national percentage of people aged who smoke increased from 2016-2018, namely from 3.39% in 2016, 3.9% in 2017 and 9.65% in 2018. Smoking habits will negatively affect human health. Cigarette smoke contains various of different chemicals that are harmful to health, including nicotine addiction, tar (carcinogen or cancer-causing), and carbon monoxide. When inhaled, they form carboxyhemoglobin with Hb (Hemoglobin). When this occurs, the amount of Hb available for oxygen transport is reduced. By way of compensation, the body will produce more red blood cells. The objective of the study was to determine the hemoglobin levels in smokers with smoking durations of 3 years, 4 years and 5 years. This study is quasi-experimental. Determine hemoglobin readings using a photometer 5010 with the Cyanmethemoglobin method. Up to 20 samples were analyzed with SPSS version 20 with Anova analysis at 95% confidence level. The results of statistical analysis showed p-value <0.05, which means that there is a significant effect between variables and vice versa. Hb level comparison between durations of smoking are of 3 years with an average value of 14.8, Hb levels of 4 years with an average value of 14.53, and Hb levels of 5 years with an average value 15.94. The results showed that there was an effect of smoking duration on the results of the examination of hemoglobin levels in smokers of 3,4 and 5 years with the greatest effect being smokers with a duration of 5 years. The duration of smoking impact on hemoglobin levels.
