BackgroundPreventive home visits (PHV) may contribute to identify risks and needs in older people, and thereby delay the onset of functional decline and illness, otherwise often followed by home care or admission to hospital or nursing homes. There is a need to increase knowledge about which factors are associated with different risk areas among older people, so that the PHV questionnaire focuses on relevant tests and questions to make the PHV more specific and have a clear focus and purpose.ObjectiveThe objective of this study was to examine associations between five kinds of risks: risk of falls, malnutrition, polypharmacy, cognitive impairment, and risk of developing illness and factors related to lifestyle, health, and medical diagnoses among older people living at home.MethodsA cross-sectional study design was applied. PHV were conducted by nurses among 77-year-old people in an urban municipality and among ≥75-year-old people in a rural municipality. A questionnaire including tests and a risk assessment score for developing illness was used. Descriptive and inferential statistics including regression models were analyzed.ResultsThe total sample included 166 persons. Poor perceived health was associated with increased risk of developing illness and risk of fall, malnutrition, and polypharmacy. Lifestyle and health factors such as lack of social support, sleep problems, and feeling depressed were associated with risk of developing illness. Risk of falls, malnutrition, polypharmacy, and cognitive impairment were also associated with increased risk of developing illness. None of the independent factors related to lifestyle, health, or medical diagnosis were associated with risk of cognitive impairment.ConclusionPoor perceived health was associated with health-related risks in older persons living at home. Preventive health programs need to focus on social and lifestyle factors and self-reported health assessment to identify older people at risk of developing illnesses.

IZVLEČEKUvod: Patronažno zdravstveno varstvo je pomemben člen pri odkrivanju, spremljanju in obravnavi kroničnih bolezni na primarni ravni. Namen raziskave je bil predstaviti izkušnje in stališča do nadgrajene preventivne obravnave ter obiskov na domu. Metode: Uporabljen je bil mešani raziskovalni dizajn. Podatki so bili zbrani z delno strukturiranim vprašalnikom (Cronbach α = 0,92). Sodelovalo je 50 diplomiranih medicinskih sester, odzivnost je bila 81-% (n = 41). Opravljena je bila tematska analiza odprtih vprašanj. Za kvantitativne podatke je bila uporabljena opisna statistika, t-test, analiza variance in korelacijska analiza. Rezultati: Prispevek preventivnih obiskov na domu anketiranci vidijo v zdravstvenovzgojnem delovanju ( = 4,34, s = 0,85), odkrivanju rizičnih dejavnikov ( = 4,12, s = 0,81) in ohranjanju kakovosti življenja pacientov ( = 4,10, s = 0,92). Zadovoljstvo anketirancev je povezano s sprejemanjem obiskov s strani pacientov in družin (r = 0,754, p < 0,001), s prispevkom k stroki patronažnega varstva (r = 0,766, p < 0,001) ter z obravnavo posameznika z družino (r = 0,693, p < 0,001). V tematski analizi sta bili identificirani dve temi, in sicer (1) pozitivne izkušnje z obiski in (2) negativne izkušnje z obiski. Diskusija in zaključek: Novo obliko preventivnih patronažnih obiskov v domačem okolju anketirani podpirajo, še posebej možnost celovite obravnave posameznika in njegove družine. Glede na ugotovitve se predlaga, da medicinska sestra v patronažnem varstvu postane enakovreden partner v timu referenčne ambulante na nacionalnem nivoju. The purpose of the study is to present the views and experience in terms of upgraded preventive treatment and home visits. Methods: The mixed research design was used. Data were collected using a semi-structured questionnaire (Cronbach α = 0.92). 50 registered nurses participated and the response rate was 81 % (n = 41). A thematic analysis of open-ended questions was done and the analysis was carried out using the PSPP tool: descriptive statistics, t-test, variance analysis, correlation analysis. Results: Respondents see an added value of preventive home visits in health education interventions ( = 4.34, s = 0.85), detection of risk factors ( = 4.12, s = 0.81) and maintaining the patients' quality of life ( = 4.10, s = 0.92). Their satisfaction is linked to the acceptance of visits by patients and families (r = 0.754, p < 0.001), benefit to the profession of community health nursing (r = 0.766, p < 0.001), and treatment of individuals with family (r = 0.693, p < 0.001). Thematic analysis highlighted two themes, namely (1) positive experiences with home visits and (2) negative experiences with home visits. Aim: To describe self-rated health in relation to lifestyle and illnesses and to identify risk factors for ill health such as pressure ulcers, falls and malnutrition among 75-year-old participants in a new clinical routine involving health assessment followed by tailored one-to-one health promotion at preventive clinic visits to a nurse at primary health care centres (PHCC). Background: There is a rapidly growing ageing population worldwide. It is central to health policy to promote active and healthy ageing. Preventive clinic visits to a nurse in primary health care were introduced as a new clinical intervention in a region in Sweden to improve the quality of health for the older adults. Design: A quantitative cross-sectional population-based study. Methods: The sample consisted of 306 individuals in six primary health care centres in Sweden aged 75 years who attended preventive clinic visits to a nurse. Data were collected from March 2014 to May 2015 during structured conversations with a nurse based on self-administered questionnaires, clinical examinations, risk assessments and after the clinic visit existing register data were collected by the researcher. Findings: Participants experienced good self-rated health despite being overweight and having chronic illnesses. Daily exercise such as walking and housework was more common than aerobic physical training. The majority had no problems with mobility but reported anxiety, pain and discomfort and had increased risk of falls. Conclusion: It is important to encourage the older adults to live actively and independently for as long as possible. The healthy older adults may benefit from the clinical intervention described here to support the individual’s ability to maintain control over their health. Such supportive assessments might help the healthy older adult to achieve active ageing, reducing morbidity and preventing functional decline.
