The Quality Standards Subcommittee of the American Academy of Neurology is charged with developing practice parameters for neurologists for diagnostic procedures, treatment modalities, and clinical disorders. The selection of topics for which practice parameters are used is based on prevalence, frequency of use, economic impact, membership involvement, controversy, urgency, external constraints, and resources required. This paper addresses the role of thymectomy in the treatment of nonthymomatous autoimmune MG.In 1939, Blalock et al. 1 reported the remission of generalized MG in a 21-year-old woman after removal of a cystic thymic tumor. Subsequently, Blalock et al.2,3 performed thymectomy on MG patients without thymoma, found hyperplasia in the thymus glands, and reported improvement in at least half of their patients. Since these reports, thymectomy, with or without the presence of thymoma, has gained widespread acceptance as a form of treatment for MG.Because a definitive study of the effectiveness of thymectomy has never been done, 4,5 the role of thymectomy in the management of MG remains uncertain. Nevertheless, physicians have to advise their patients regarding the benefits of thymectomy based on the existing literature. Our goal is to develop evidence-based recommendations for clinicians considering thymectomy for patients with nonthymomatous autoimmune MG by performing a systematic review and analysis of the literature. Process. Identification and selection of studies.We searched the National Library of Medicine's Medline database from 1966 to February 1998 using the medical subject headings "myasthenia gravis" (restricted to the surgery subheading) and "thymectomy." To identify articles published before 1966, or missed by our original search strategy, we reviewed the references of the identified articles.We classified the resulting articles into the following categories: Class II, controlled but nonrandomized studies describing outcomes in MG patients with and without thymectomy; and Class III, uncontrolled case series describing outcomes in patients with or without thymectomy.Within a given controlled (Class II) study, we planned to describe the therapeutic effect of thymectomy by comparing the rate MG patients receiving thymectomy achieved desirable outcomes with the rate MG patients not receiving thymectomy achieved desirable outcomes. The nonrandomized nature of Class II studies often introduces differences in the baseline characteristics of the thymectomy and nonthymectomy patient groups that can effect MG outcomes independent of thymectomy. To the extent possible, we planned to adjust for these confounding differences in baseline prognostic characteristics. The thymus is believed to play a central role in the pathogenesis of Myasthenia gravis (MG). According to a previous hypothesis, MG is initiated within the thymus by immunogenic presentation of locally produced nicotinic acetylcholine receptor (AChR) to potentially autoimmune T cells. Data of 10 consecutive MG patients demonstrate two critical features of MG thymuses that support the concept of intrathymic activation of autoreactive, AChR-specific lymphocytes. Morphologically, the thymuses showed lympho-follicular hyperplasia in nine cases and benign thymoma in one case. The paramount feature revealed by immunohistological double marker analyses was the intimate association of myoid cells (antigen producing) with interdigitating reticulum cells (potentially antigen presenting cells), both of which were surrounded by T3+ lymphocytes in thymus medulla. All 10 thymuses contained T lymphocytes reactive with AChR. This was in contrast to the peripheral immune compartment (blood) where in only 3 of 10 patients, significant T cell responses to AChR were observed. AChR-specific T cell lines could be established from 8 of 10 thymuses, all members of the helper/inducer subset as indicated by the expression of markers T3 and T4.

Hatás:A budesonid hatóanyag a prednisolon-sorba tartozó, nem halogénezett glukokortikoid. Lo kális hatáserőssége a halogénezett származékokéval azonos. Javallat:Kortikoszteroidra reagáló gyulladásos bőrbetegségek, elsősorban atópiás dermatitisz, pszoriázis, ekcéma. Kenőcsből a felszívódás lassúbb, mint a krém gyógyszerformából, hatása tovább tart, ezért krónikus folyamatok kezelésére előnyben részesítendő. Ellenjavallat:Vírusos, bakteriális, gombás fertőzés a bőrön; bőrtuberkulózis; szifiliszes bőrelváltozás; herpes simplex; rosacea; perioralis dermatitis; túlérzékenység va lamelyik komponenssel szemben. Terhesség, laktáció relatív ellenjavallatot képez, mérlegelni kell a várható terápiás előnyt és kockázatot, mert átjuthat a placentán és kiválasztódik az anyatejben. Alkalmazás:Az érintett területet naponta 1-2 alkalommal vékonyan bekenni. Fenntartó kezelés esetén elegendő a napi egyszeri alkalmazás. Mellékhatás:Zárt fedőkötés gyorsítja a hatóanyag felszívódását, ezért ilyen kötéssel, valamint nagy felü leten alkalmazva felszívódhat és szisztémás kortikoszteroid hatást fejthet ki. Ritkán a zárt kötés alatt bevérzések is jelentkeznek. Sokáig azonos területen alkalmazva, különösen fiatal egyéneken, bőratrófia alakulhat ki. Másodlagos szuperinfekció, acneiform eruptio, irritatív dermatitis, égő érzés, viszketés, a bőr kiszáradása, folliculitis, hypopigmentáció, striae, hypertrichosis, arcbőrön papulózus dermatitisz esetleg előfordulhat. Flgyelmeztetós:A kezelés időtartama a klinikai eredménytől függ. Négy héten túli kezelés csak a beteg álla potának alapos mérlegelésével végezhető. Fertőzések esetén megfelelő antibakteriális, il letve antifungális kezelést kell folytatni. Bőrirritáció vagy szenzibilizáció esetén a kezelést abba kell hagyni. Gyermekek kezelése megfontolást és fokozott körültekintést igényel. Szem és szemkörnyéki területek, valamint nyálkahártya kezelésére nem alkalmazható!!
