Effects of photodynamic therapy on the choriocapillaris and retinal pigment epithelium in the irradiated area

Abstract: Although the RPE in the irradiated area was preserved, 12 months after PDT, mild choriocapillaris occlusion was detected in the irradiated area in eyes that had undergone multiple PDT sessions.

“…In MONT BLANC and DENALI, patients in the combination arms received a mean of 1.7-1.9 verteporfin PDT treatments [15,16]. In patients treated with combination therapy, multiple treatments with verteporfin PDT have been associated with reduced VA gains compared with monotherapy and may explain the lower VA results of these previous studies compared with ours [21]. …”

“…During the initial 3 months, BCVA improvements were delayed in the combination therapy group compared with the monotherapy group; however, by month 6, there was no difference between the groups. The slower improvement in BCVA in the combination therapy group may have resulted from hypoperfusion of the choriocapillaris caused by verteporfin PDT [21]. In both treatment groups, reductions in CRT were paralleled by improvements in BCVA.…”

“…Mild choriocapillaris occlusion remained in the irradiated area in eyes that had undergone multiple PDT sessions 12 months after the initial PDT. 23 Hypoxia may accelerate the expression of vascular epithelial growth factor (VEGF). PDT may alter the natural progression of PCV in the narrow sense, causing enlargement or a change to polypoidal CNV as a result of ischemia and increased VEGF expression.…”

“…Dewi et al suggest that RPE atrophy is the natural course of CNV development 18 . Unfortunately, due to the absence of pretreatment evaluation of RPE this finding is limited.…”

“…We analysed the area of RPE atrophy after 12 months of follow-up. The analysis was done using a method described by Dewi et al using a hyperflorescence area without leak in late phase of florescein angiograms 18 . The measurement was performed using Topcon i-base software (version 3.8.1.)…”
