Background: Periodontitis is result of cumulative exposure of dental plaque, thus the main aim of periodontal therapy is the prevention of plaque accumulation and maintain periodontal health. The clinical effect of scaling and root-planning (SRP) are well documented .Antimicrobial agents act as an adjunct to periodontal therapy. One of the most frequently used antimicrobial agents is chlorhexidine gluconate (CHX), it is a broad spectrum antiseptic with a pronounced antimicrobial effect Clinical improvements after SRP are associated with microbiological changes that include a decrease in microbial load and a mean percentage change of certain periodontal pathogens , such as Treponema denticola, Porphyromonas gingivalis and Tarnella forsythus. These species are gram negative anaerobes which possess, an enzyme capable of hydrolyzing synthetic trypsin substrate, BANA. Methods: This study included 30 individuals who were randomly distributed in two groups test (SRP + CHX) and control (SRP) Results: The results of the study stated that the treatment with SRP and CHX improved clinical and microbiological parameters compared to the SRP alone as a monotherapy. Conclusion: CHX rinsing and repeated professional plaque removal could have equivalent therapeutic benefits, the use of CHX offers the great advantage of not requiring the patient's presence in the dental office.

Efecto de las enterobacterias en pacientes con periodontitis crónicaEffect of enterobacteriaceae in patients with chronic periodontitis ARDILA MEDINA CM* RESUMENLa periodontitis crónica es una enfermedad infecciosa asociada a microorganismos gram-negativos anaerobios. Esta entidad de naturaleza crónica y de alta prevalencia en la población, ha sido relacionada con un riesgo aumentado a infarto agudo del miocardio, accidente cerebrovascular, bajo peso al nacer y parto pretérmino. Estudios microbiológicos en pacientes con periodontitis en diferentes partes del mundo han mostrado altas prevalencias de sobreinfección por enterobacterias, las cuales podrían complicar el cuadro clínico de los pacientes y la respuesta a la terapia periodontal. Así mismo, el lipopolisacári-do presente en su pared, ha demostrado estimular la activación celular y la producción de citoquinas proinflamatorias que podrían aumentar el riesgo de enfermedades sistémicas en estos pacientes. Aunque la terapia periodontal reduce los marcadores inflamatorios en suero y ha mostrado una reducción en la incidencia de bajo peso al nacer y parto pretérmino, no se ha estudiado el efecto de las enterobacterias a nivel subgingival sobre la respuesta a la terapia básica y los efectos sisté-micos que pueda desencadenar. El objetivo de esta revisión es proporcionar fundamentos que permitan de alguna manera dilucidar tal problemática. PALABRAS CLAVE:Periodontitis, microbiología, enterobacterias, patógenos. SUMMARYThe chronic periodontitis is an infectious disease associated to anaerobic gram negative microorganisms. This organization of chronic nature and high prevalence in the population has been associated with a risk increased to acute infarct of the myocardium, stroke, low birth weight and preterm infants. Microbiological studies in patients with periodontitis in different parts from the world have shown high prevalences of sobreinfection by enterobacteriaceae, which could complicate the clinical picture of the patients and the answer to the periodontal therapy. Also, the present lipopolisacaric one in its wall, has demonstrated to stimulate the cellular activation and the production of proinflammatory cytokines that could increase the risk of systemic diseases in these patients. Although the periodontal therapy reduces the inflammatory serum markers and has shown a reduction in the incidence of low birth weight and preterm infants, has not studied the effect of enterobacteriaceae at subgingival level on the sistemic answer to the basic therapy and effects that can trigger. The objective of this revision is to provide foundations that somehow allow to explain so problematic.

Supragingival biofilm control and systemic inflammation in patients with type 2 diabetes mellitus

The objective of this study was to evaluate the effect of strict supragingival biofilm control on serum inflammatory markers and on periodontal clinical parameters in type 2 diabetes mellitus (T2DM) patients with chronic severe periodontitis. Twenty-four individuals with T2DM and periodontitis were randomly allocated to two treatment groups. The supragingival therapy group (ST, n = 12) received supragingival scaling, whereas the intensive therapy group (IT, n = 12) underwent supra-and subgingival scaling, as well as root planing. Patients from both groups received professional oral hygiene instructions every month. Data regarding visible plaque index (VPI), gingival bleeding index (GBI), bleeding on probing (BOP), probing pocket depth (PPD), clinical attachment level (CAL), serum levels of interleukin (IL)-6, IL-17A, IL-8, tumor necrosis factor α (TNF-α), monocyte chemoattractant protein (MCP)-1 enzyme-linked immunosorbent assay (ELISA), and glycated hemoglobin (HbA1c) levels were obtained at baseline and at 6 months post-therapy. Both therapies resulted in the improvement of almost all clinical periodontal parameters (p < 0.05). There were no differences in TNF-α, IL-8, IL-17A and HbA1c levels in either group (p > 0.05), between the two periods. However, MCP-1 levels were significantly reduced in both the ST (p = 0.034) and the IT (p = 0.016) groups, whereas the serum IL-6 levels were significantly reduced only in the IT group (p = 0.001). Strict control of supragingival biofilm has a limited effect on systemic inflammatory markers, and a moderate effect on periodontal clinical parameters.

Abstract: In Brazilians with untreated chronic periodontitis, SRP led to clinical improvement associated with a decrease of certain periodontal pathogens, and an increase of beneficial species for up to 9 months after therapy.
