Effects of nasal continuous positive airway pressure (NCPAP) on breathing pattern in spontaneously breathing premature newborn infants

Abstract: NCPAP improves the breathing strategy of premature infants with respiratory failure, as reflected by improved thoraco-abdominal synchrony, increased Vt and reduction of the LBI. This effect is associated with an increase in EELV-level with CPAP level. However, further investigations are necessary to establish the best CPAP level that ensures both safety and efficiency.

Studies show a distending pressure is important for maintaining FRC (48), increasing lung compliance and improving oxygenation, and that 8 cm H 2 O is more effective than a lower pressure (49). Studies have used pressures up to 10 cm H 2 O (50) and Gregory et al(51) used a nCPAP pressure up to 12 mm Hg.…”

“…In the infants on CPAP there was an increased EEL at 2 h which corresponded with improved homogeneity and increased regional ventilation of the posterior lung. This is not surprising as it is widely acknowledged that CPAP increases EEL (32)(33)(34). It has been speculated that the dynamic volumepreserving mechanisms resulting from expiratory flow braking are not required during CPAP, as the constant pressure passively elevates functional residual capacity (30,32).…”

“…As metilxantinas e a cafeína são os fármacos mais comumente utilizados. A CPAP nasal e mais recentemente a VNPPI também encontram-se bem estabelecidas no tratamento da ADP 65,66 . Infelizmente, nenhum estudo randomizado foi realizado para comparar o tratamento farmacológico com o não-farmacológico da ADP.…”
