Introdução: condromalácia patelar é uma condição ortopédica que deteriora e amolece a articulação da patela. Introdução: Condromalácia Patelar (CMP) é uma condição ortopédica que deteriora e amolece a articulação da patela. Objetivo: apresentar os testes clínicos e funcionais utilizados no tratamento da CMP, verificando as condutas fisioterapêuticas para pacientes diagnosticados com essa lesão de joelho. Método: trata-se de uma revisão integrativa com as recomendações da estratégia PICO e escala PEDro. Foi explorada a base de dados PubMed, com demarcação temporal entre 2017 e 2023. Resultados: foram incluídos 12 artigos que avaliaram nos testes clínicos: dor, capacidade funcional, desempenho e equilíbrio. Conclusão: as condutas fisioterapêuticas envolvem: treinamento neuromuscular, alongamento, mobilizações, tração manual, fortalecimento do quadríceps, fortalecimento e estabilização do quadril e do joelho.

Anterior Knee Pain: State of the Art

Anterior knee pain (AKP) is one of the most common conditions to bring active young patients to a sports injury clinic. It is a heterogeneous condition related to multiple causative factors. Compared to the general population, there appears to be a higher risk of development of patellofemoral osteoarthritis in patients with AKP. AKP can be detrimental to the patient’s quality of life and, in the larger context, significantly burdens the economy with high healthcare costs. This study aims to present a comprehensive evaluation of AKP to improve clinical daily practice. The causes of AKP can be traced not only to structures within and around the knee, but also to factors outside the knee, such as limb malalignment, weakness of specific hip muscle groups, and core and ligamentous laxity. Hence, AKP warrants a pointed evaluation of history and thorough clinical examination, complemented with relevant radiological investigations to identify its origin in the knee and its cause. Conservative management of the condition achieves good results in a majority of patients with AKP. Surgical management becomes necessary only when it is deemed to provide benefit—when the patient has well-characterized structural abnormalities of the knee or limb that correlate with the AKP clinically or in situations where the patient does not obtain significant or sustained relief from symptoms. AKP has a multifactorial etiology. The treatment strategy must be individualized to the patient based on the patient profile and specific cause identified. Hence, treatment of AKP warrants a pointed evaluation of history and thorough clinical examination complemented with relevant radiological investigations to identify the condition’s origin and its cause. A holistic approach focused on the patient as a whole will ensure a good clinical outcome, as much as a focus on the joint as the therapeutic target.

PurposeThe purpose of this study was to develop a multidisciplinary guideline for patellofemoral pain (PFP) and patellar tendinopathy (PT) to facilitate clinical decision‐making in primary and secondary care.MethodsA multidisciplinary expert panel identified questions in clinical decision‐making. Based on a systematic literature search, the strength of the scientific evidence was determined according to the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) method and the weight assigned to the considerations by the expert panel together determined the strength of the recommendations.ResultsAfter confirming PFP or PT as a clinical diagnosis, patients should start with exercise therapy. Additional conservative treatments are indicated only when exercise therapy does not result in clinically relevant changes after six (PFP) or 12 (PT) weeks. Pain medications should be reserved for cases of severe pain. The additional value of imaging assessments for PT is limited. Open surgery is reserved for very specific cases of nonresponders to exercise therapy and those requiring additional conservative treatments. Although the certainty of evidence regarding exercise therapy for PFP and PT had to be downgraded (‘very low GRADE’ and ‘low GRADE’), the expert panel advocates its use as the primary treatment strategy. The panel further formulated weaker recommendations regarding additional conservative treatments, pain medications, imaging assessments and open surgery (‘very low GRADE’ to ‘low GRADE’ assessment or absence of scientific evidence).ConclusionThis guideline recommends starting with exercise therapy for PFP and PT. The recommendations facilitate clinical decision‐making, and thereby optimizing treatment and preventing unnecessary burdens, risks and costs to patients and society.Level of EvidenceLevel V, clinical practice guideline.
