There is a lack of knowledge about the effectiveness of manual therapy (MT) on subjects with temporomandibular disorders (TMD). The aim of this systematic review is to synthetise evidence regarding the isolated effect of MT in improving maximum mouth opening (MMO) and pain in subjects with signs and symptoms of TMD. MEDLINE(®) , Cochrane, Web of Science, SciELO and EMBASE(™) electronic databases were consulted, searching for randomised controlled trials applying MT for TMD compared to other intervention, no intervention or placebo. Two authors independently extracted data, PEDro scale was used to assess risk of bias, and GRADE (Grading of Recommendations Assessment, Development and Evaluation) was applied to synthetise overall quality of the body of evidence. Treatment effect size was calculated for pain, MMO and pressure pain threshold (PPT). Eight trials were included, seven of high methodological quality. Myofascial release and massage techniques applied on the masticatory muscles are more effective than control (low to moderate evidence) but as effective as toxin botulinum injections (moderate evidence). Upper cervical spine thrust manipulation or mobilisation techniques are more effective than control (low to high evidence), while thoracic manipulations are not. There is moderate-to-high evidence that MT techniques protocols are effective. The methodological heterogeneity across trials protocols frequently contributed to decrease quality of evidence. In conclusion, there is widely varying evidence that MT improves pain, MMO and PPT in subjects with TMD signs and symptoms, depending on the technique. Further studies should consider using standardised evaluations and better study designs to strengthen clinical relevance.

Introduction: Temporomandibular Disorder (TMD) is caused by multiple factors and exhibits various symptoms. Its most common symptom is pain, a factor that can in luence the quality of an individual's sleep. Objective: The aim of the present study was to assess the correlation between TMD and sleep. Methods: A cross-sectional study was conducted with 60 women, aged between 18 and 40 years, who had been diagnosed with TMD based on the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Fonseca's anamnestic index (FAI) was used to classify the severity of the dysfunction. To complete the assessment, each volunteer was assessed using the visual analogue scale (VAS) for pain. They also responded to two questions related to their quality of sleep and the average hours of sleep per night. Results: The more severe the TMD, the fewer hours of sleep per night (r = -0.5663). Ö ÖZ ZE ET T Toplumda yüksek oranda gözlemlenen temporomandibuler eklem (TME) rahatsızlıkları çene, boyun ve baş ağrıları, eklem sesleri gibi pek çok semptomla bireylerin yaşam kalitesini düşürmek-tedir. Bu rahatsızlıklar tedavi edilmediği takdirde ilerlemekte ve TME'de kalıcı hasara sebep olabilmektedir. TME rahatsızlıkları için pek çok tedavi yöntemi bildirilmiştir. Bu tedavi yöntemleri geri dönüşümlü (noninvaziv) ve geri dönüşümsüz (invaziv) tedaviler olmak üzere ikiye ayrılmıştır. Geri dönüşümlü tedaviler arasında bilgilendirme yöntemi, fizik tedavi yöntemleri, farmakolojik tedavi, psikiyatrik destek tedavisi ve oklüzal splint tedavileri yer alır iken; geri dönüşümsüz tedaviler oklüzal uyumlama ve cerrahi girişimleri içermektedir. TME rahatsızlıklarında geri dönüşümsüz tedavilerden önce daha konservatif yöntemler olan geri dönüşümlü tedaviler denenmesi temel prensiptir. Tavsiye edilen tedavi yöntemlerinin etkinliklerini araştırmak amacıyla pek çok çalışma yapılmıştır. Bununla birlikte, tedavi etmede ya da semptomları azaltmada hangi tedavi yönteminin ya da hangi splint tipinin daha etkin olduğuna dair kesin bir kanıt mevcut değildir. Etki mekanizmaları ve tedavi başarıları tartışmalı olmasına rağmen, TME rahatsızlıkları tedavisinde en çok kullanılan yöntem oklüzal splint tedavisidir. Diş hekimliğinde her biri farklı özel durumlar için olmak üzere birçok oklüzal splint kullanılmaktadır. Splint endikasyonları TME inflamasyonu, eklem içi dü-zensizlik, cerrahi sonrası tedavi, hipertonik yüz kasları, diş sıkma ve gıcırdatma olarak belirtilmektedir. These treatment methods are divided into two as non-invasive treatments and invasive treatments. Non-invasive treatments include informative methods, physical therapy, pharmacological treatment, psychiatric support therapy and occlusal splint therapies and invasive treatments include occlusal adjustment and surgical interventions.
