Effects of ionizing irradiation and interface backscatter on human mesenchymal stem cells cultured on titanium surfaces

Abstract: Radiotherapy to the head and neck region negatively influences the osseointegration and survival of dental implants. The effects of cobalt 60 (60Co) ionizing radiation and the impact of backscatter rays were investigated on human mesenchymal stem cells cultured on titanium surfaces. Bone marrow‐derived human mesenchymal stem cells were seeded on titanium (Ti), fluoride‐modified titanium (TiF), and tissue culture plastic. Cells were exposed to ionizing γ‐radiation in single doses of 2, 6, or 10 Gy using a 60Co … Show more

“…The IL-6 transcripts were upregulated at 3 up to 14 days in the IrI and IrL groups. IL-6 was secreted by hMSCs at day 1 after irradiation and started declining at day 7 until day 21, regardless of the dose and implant surface [29]. The release of IL-6 and GM-CSF contributes to pathological bone resorption by stimulating the differentiation of mononuclear phagocyte osteoclast precursors into mature bone-resorbing cells [10,30].…”

“…In bone marrow, PGE2 production has been reported not to reach a maximum until several days after irradiation [31] as we demonstrated. A viable stem cell will naturally produce high levels of immune-response factors after any trauma or external influence, such as irradiation [29].…”

“…Endostatin can specifically act on endothelial cells during neovascularization. It can also have an antiangiogenic effect by regulating the expression of VEGF and the activity of proteolytic enzymes on the surface of tumor cells, which indirectly leads to tumor dormancy or retraction[ 15 , 16 ]. The advantage of antiangiogenic therapy is that it can be targeted toward pathological blood vessels and tumor blood vessels, and resistance to antiangiogenic therapy does not easily develop, mainly because the vascular endothelial genome is relatively stable.…”

“…Since then, several clinical trials have reported positive results on the primary placement protocol [ 30–33 ]. However, precautions must be taken as radiation backscatter from titanium has been shown to escalate the radiation dose to the cells growing on the surface with more than 40% [ 34 ]. Fortunately, backscattered radiation from titanium is of a very short range and cells at a distance of 1 mm from the surface receive only a 15% higher radiation dose due to backscatter [ 34 ].…”

“…Osteoblasts play an essential role in bone regeneration and remodeling, and thus, in the osseointegration of dental implants [ 35 ]. While a previous report showed that the escalated radiation dose caused by backscatter from titanium did not influence the response of human mesenchymal stem cells significantly [ 34 ], OBs have demonstrated a radiation dose-dependent reduction in cell attachment and proliferation [ 36 , 37 ]. However, in vitro studies investigating the effects of irradiation on human OBs cultured on titanium are very limited.…”
