“…One of several proposed etiologies is that residual incongruities (i.e., anatomical irregularities in the articular surfaces) remaining after the injury and treatment lead to aberrations in the joint contact stress distributions which, over time, predispose to post-traumatic OA (Lefkoe et al, 1993;Wagner et al, 1996). The FE method provides an ideal vehicle by which to study joint contact stresses on a patient-specific basis.…”

“…To highlight this matter I will like to remind the reader that previous clinical studies support the opinion that a step-off of more than 2 mm in the radiocarpal joint is associated with increased arthritic changes (Knirk and Jupiter, 1986), while steps of 1 mm cause pain and decreased range of motion and grip strength (Fernandez and Geissler, 1991;Trumble et al, 1994). Furthermore, biomechanical studies (Wagner et al, 1996) have shown that 1 mm steps cause significant pressure changes. This study showed that scaphoid facet depression produced more overload on the intact area than lunate facet depression (die punch fragment).…”

“…47 This in part is caused by overload on the unaffected fossa 48 and because the malunited fragments erode the opposing surface of the carpal bone. 47,49 Early osteotomy for impending malunion is advised and good results have been reported.…”
