Effects of immunosuppression on immune response to pneumococcal vaccine in inflammatory bowel disease: A prospective study*

Abstract: Anti-TNF therapy alone or in combination with azathioprine impairs the response to pneumococcal vaccination in patients with IBD. All patients with IBD should therefore be vaccinated before starting anti-TNF therapy.

“…Assessing the amount of anti-pneumococcal antibodies, the infliximab group showed a 2.69-fold increase in antibody titer, compared with a 5.71-fold increase in the control group. 64 These results were reaffirmed in a recent prospective study of Korean CD patients, which found that the female sex and anti-TNF therapy were significant nonresponse predictors to pneumococcal polysaccharide vaccination (P = 0.015, P = 0.048, respectively). 65 PCV-13 vaccine, in which polysaccharides are conjugated to an immunogenic carrier protein, is expected to be more immunogenic than PPV23.…”

“…Die Pneumokokkenimpfung zeigt bei nicht immunsupprimierten IBD-Patienten ein normales Ansprechen, bei immunsupprimierten Patienten, insbesondere jenen unter TNFα-Blockern ist das Ansprechen jedoch signifikant geringer [125]. Deshalb sollten diese Patienten möglichst vor Beginn der ISP gegen Pneumokokken geimpft werden [114].…”

“…While data concerning the need for revaccination with conjugated vaccine are scant, PPSV23 revaccination after 5 years is recommended for immunocompromised patients [34] . However, the response to Streptococcus pneumoniae vaccinations may be impaired in IBD patients treated with immunomodulators, particularly when they are used in combination [35,36] . For this reason, it would be advisable to perform vaccinations for pneumococcal infections before starting immunosuppressive drugs.…”
