Effects of Hypothermia vs Normothermia on Societal Participation and Cognitive Function at 6 Months in Survivors After Out-of-Hospital Cardiac Arrest

Gisela Lilja,
Susann Ullén,
Josef Dankiewicz
et al.

Abstract: ImportanceThe Targeted Hypothermia vs Targeted Normothermia After Out-of-Hospital Cardiac Arrest (TTM2) trial reported no difference in mortality or poor functional outcome at 6 months after out-of-hospital cardiac arrest (OHCA). This predefined exploratory analysis provides more detailed estimation of brain dysfunction for the comparison of the 2 intervention regimens.ObjectivesTo investigate the effects of targeted hypothermia vs targeted normothermia on functional outcome with focus on societal participatio… Show more

“…Based on HADS scale, the prevalence of anxiety and depression among OHCA survivors are previously reported to be about 20–30% 28 . Even though the screening tools that we used were not validated in patients with OHCA, the telephone versions of Mini MoCA and AD-8 for cognitive impairment and PHQ-9 and HADS for mood disorders might be feasible OHCA screening tools for long-term follow-up 30 . However, with regard to cognitive test, AD-8 could yield too many false positives when compared to a performance-based tool such as the Mini MoCA and should be interpreted cautiously.…”

“…Global cognitive function was evaluated using MoCA according to the critiques and recommendations set by the International Parkinson and Movement Disorders Society ( 18 ). The overall MoCA score ranges from 0 to 30, with a value below 26 indicating CI ( 20 ). The participants were then classified into two groups: PD patients with normal cognition (PD-NCI) and PD patients with CI (PD-CI).…”

“…At Day 90, following randomisation, site research coordinators contacted participants by phone to check whether hospital readmission had occurred, whether there had been nonprotocol study drug administration, any adverse events had occurred, and conduct a quality-of-life survey using EuroQuol 5D-5L (EQ5D5L) and a cognitive function survey using the telephone version Montreal Cognitive Assessment (T-MOCA). 23 As part of the process of consumer input into the pivotal trial design, all pilot participants were asked to consider the minimum number of additional days at home until day 90 that would be of value to them. This was recorded as the participant-reported DAH 90 minimum clinically important difference.…”
