We evaluated access to primary health care for older women and men in Barbados, a developing country, using a widely accepted access indicator, hospitalization for ambulatory care sensitive conditions. Using 2003-2008 data, we calculated gender-specific total annual population-based rates of these hospitalizations per 1,000 older women and men and individual rates for the six most prevalent conditions. Across the 6 years, these hospitalizations increased 33.6% for women, 30.6% for men (both P < .0001). However, the average rate for diabetes fell 32% for women, 36% for men. Findings suggest an opportunity to improve access to primary health care, particularly for older women.

 ABSTRACT: Objective: The objective of this article is to estimate the probabilities of transition between different degrees of functional dependence, as well as the predictive capacity of this dependence for general mortality of women and men living in Mexico aged 65 and over. Methods: By means of probabilities, transitions between different degrees of functional dependence are calculated and, with proportional Cox risks models, the relative risks of mortality are estimated. Results: The results indicate that women are more likely to move from mild functional dependence to moderate or severe functional dependence than men. Likewise, women and men with severe functional dependence, assessed at the baseline, have higher relative risks of mortality than the mortality risks of individuals without functional dependence. Conclusion: There are differences in the probabilities of transition to functional dependence, noting that women are more likely to move to higher degrees of physical restraint than men. In this sense, having physical limitations in the development of basic activities of daily living increases mortality risks among Mexican elderly.

Estimating health expectancy in presence of missing data: an application using HID survey

In this article we estimate health transition probabilities using longitudinal data collected in France for the survey on handicaps, disabilities and dependencies from 1998 to 2001. Life expectancies with and without disabilities are estimated using a Markov-based multi-state life table approach with two non-absorbing states: able to perform all activities of daily living (ADLs) and unable or in need of help to perform one or more ADLs, and the absorbing state of death. The loss of follow-up between the two waves induces biases in the probabilities estimates: mortality estimates were biased upwards; also the incidence of recovery and the onset of disability seemed to be biased. Since individuals were not missing completely at random, we correct this bias by estimating health status for drop-outs using a non parametric model. After imputation, we found that at the age of 70 disability-free life expectancy decreases by 0.5 years, whereas the total life expectancy increases by 1 year. The slope of the stable prevalence increases, but it remains lower than the slope of the cross sectional prevalence. The gender differences on life expectancy did not change significantly after imputation. Globally, there is no evidence of a general reduction in ADL disability, as defined in our study. The added value of the study is the reduction of the bias induced by sample attrition.
