Effects of Argon Laser Trabeculoplasty in the Early Manifest Glaucoma Trial

Abstract: Methods: Patients randomized to the treatment arm of the EMGT received a standard treatment protocol (topical betaxolol hydrochloride followed by 360°ALT) and were then followed prospectively at 3-month intervals for up to 8 years. One eye per patient was included in the analyses. We investigated the relationship between pre-ALT IOP and subsequent IOP reduction, and other factors that might have influenced the effect of ALT, including stage of the disease, trabecular pigmentation, presence of exfoliation syndr… Show more

“…Similar to this finding, Heijl et al. () showed in the EMGT that each 3 mmHg higher IOP value before ALT was associated with an additional mean IOP reduction of approximately 2 mmHg after ALT. This might be important as every mmHg IOP reduction slows progression by 10 per cent as was shown in the EMGT (Leske et al.…”

“…Heijl et al. () reported similar findings using argon laser trabeculoplasty (ALT) in the Early Manifest Glaucoma Trial (EMGT). Therefore, the indication for SLT has to be put cautiously in patients with low pre‐SLT IOPs.…”

“…Laser trabeculoplasty, drainage implants (i.e., seton/tube/shunt) surgery or ciliary body ablation have all been used and provide shorter or longerterm benefit but are all associated with moderate risks [13,14]. Newer techniques that hold potential as surgical options in primary open-angle glaucoma include deep sclerectomy/viscocanalostomy with or without collagen implant and 360-degree suture canaloplasty are being evaluated.…”

“…6 Although laser and surgical treatment options are available for this purpose, topical medical treatment is the most frequent and the most preferred method. [7][8][9] Patients use one or several eye drops regularly every day. 10 Majority of glaucoma patients instill eye drops by keeping their heads in the hyperextension position.…”
