Effects of Acupuncture Treatment on Fibromyalgia Symptoms, Serotonin, and Substance P Levels: A Randomized Sham and Placebo-Controlled Clinical Trial

Abstract: Acupuncture, rather than sham or placebo acupuncture, may lead to long-term improvements on clinical outcomes and pain neuromediator values. Changes in serum serotonin and SP levels may be a valuable explanation for acupuncture mechanisms in FM treatment.

“…There is also evidence that acupuncture potentiates the effect of antidepressants and exercise on pain by up to 30%, with poor, almost nil, adverse effects [17]. Regarding analgesic effects, verum acupuncture, rather than sham or placebo acupuncture, lead to changes in serum serotonin, neuropeptide Y and SP levels that may be contributing to long-term improvements on clinical outcomes in FM treatment [54,55]. In our study, the higher baseline VAS score associated to the most severe patients, usually more resistant to treatments, emphasized the efficacy of acupuncture in this setting.…”

“…We believe that this approach can be a great help mainly for patients, but also for clinicians, providing them with a proposal on the modalities of prescription of acupuncture. e effectiveness of acupuncture in FMS is well known [3][4][5][6][7][8]. An interesting study published in 2008 and conducted by Brazilian researchers randomized 58 female patients to 20 acupuncture sessions in addition to exercise and tricyclic antidepressants versus exercise and tricyclic antidepressants.…”

“…Cognitive behavioral therapy (CBT) interventions may slow or reverse cortical gray matter atrophy, diminishescirculating proinflammatory cytokines (IL-6, IL-8, and TNF-α level) of FM patients, pain symptoms and pain perceptions, helps FM patient having fear of pain, anxiety, depression and sleep disturbances [282][283][284][285][286][287]. Physiotherapy and acupuncture, both are compelling, not found to be more beneficial than each other; longer post-treatment followup may help arbitratethe superior treatment option [288]. Low to moderate intensity endurance and strength training are strongly suggested in FM patients.…”
