Effects of a Structured Exercise Program on Physical Activity and Fitness in Colon Cancer Survivors: One Year Feasibility Results from the CHALLENGE Trial

Abstract: Background: There is strong interest in testing lifestyle interventions to improve cancer outcomes; however, the optimal methods for achieving behavior change in large-scale pragmatic trials are unknown. Here, we report the 1-year feasibility results for exercise behavior change in the Canadian Cancer Trials Group CO.21 (CHALLENGE) Trial. Methods: Between 2009 and 2014, 273 high-risk stage II and III colon cancer survivors from 42 centers in Canada and Australia were randomized to a structured exercise program … Show more

“…The Colon Health and Life‐Long Exercise Change (CHALLENGE) Trial is the first phase 3 trial examining the effects of a 3‐year structured PA program on disease‐free survival in patients with stage II and III colon cancer who have recently completed chemotherapy . To date, the trial has demonstrated feasibility in accrual and PA behavior change; it has randomized over 590 of the planned 962 patients. The Intense Exercise for Survival (INTERVAL) Trial is another phase 3 trial examining the effects of a 2‐year structured PA program on overall survival in 866 men with metastatic, castrate‐resistant prostate cancer .…”

“…More recently, hybrid programs that are able to support sustained PA have emerged. Generally, these programs begin with an on‐site, supervised phase and then taper to an off‐site phase (eg, the CHALLENGE trial) …”

“…Researchers have developed a 3‐year RCT exercise protocol in colon cancer, with supervised sessions, behavioral support and health education for the intervention group and health education only for the control . A 1‐year feasibility report has so far been successful . This controlled design, uniquely combining structured activity and motivational support, may provide the basis for a similar protocol in AIMSS.…”

“…We were delighted to read the recent report by Courneya and colleagues on the 1-year feasibility and fitness benefits of a structured exercise program among colon cancer survivors in the CHALLENGE Trial (1). These findings, and the expected 3-year follow-up data, are an important contribution to the nascent body of research examining the effectiveness of sustained exercise behavior change in cancer patients.…”
