ÖzetAmaç: Bu çalışma osteoartrit (OA)'li hastaların evde egzersiz yapma ve egzersizden yarar görme durumlarını belirlemek amacıyla tanımlayıcı olarak yapıldı. Gereç ve Yöntem: Çalışma Fizik Tedavi ve Rehabilitasyon (FTR) polikliniğine başvuran, OA tanısı konulan, kurum izni alınan ve çalışmaya katılmayı kabul eden 103 hasta ile tamamlandı. Araştırmanın verileri literatür taranarak oluşturulan anket formuyla FTR polikliniğindeki hastalarla yüz yüze görüşme tekniği kullanılarak yapıldı. Verilerin değerlendirilmesinde ki-kare önemlilik testi ve lojistik regresyon analizi kullanıldı. p<0,05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Katılımcıların %58,3'ünün egzersiz yaptığı ve egzersiz yapanların %80'inin haftada üç defa kas germe ve eklem egzersizleri yaptıkları saptandı. Egzersiz yapanların %66,7'si yaptığı egzersizin OA'ya bağlı ağrı, sabah tutukluğu vb. şikayetlerini azalttığını belirtti. Hastaların en fazla ağrı (%77,6) olmak üzere, egzersizin etkinliğine inanmama, zaman bulamama, yeterince bilgisinin olmaması gibi nedenlerle düzenli egzersiz yapmadıkları belirlendi. OA'lı hastaların egzersiz yapmasına bilgi alma durumunun çok ileri düzeyde (p<0,001), tutulan eklem türü ve gelir durumunun anlamlı düzeyde (p<0,01) etkisi olduğu bulundu. Metarials and Methods: The research was carried out on 103 patients who had been admitted to the physical therapy and rehabilitation (PMR) clinic with the diagnosis of OA, and who agreed to participate in the study, having been granted permission by the institution. The research data were obtained using a questionnaire, formulated on the basis of a literature review, which was completed by patients at the PMR clinic, using a face-to-face interview technique. In the assessment of data, the chi square test and logistic regression analysis were used. p value less than 0.05 was considered statistically significant. Results: It was found that 58.3% of participants exercised at home, and that 80% of those who exercised did muscle and joint exercises three times a week. Sixty-six point seven percent of those who exercised stated that exercise reduced symptoms such as pain, morning stiffness caused by OA. It was established that the reasons for patients not taking regular exercise included principally pain (76% of them), disbelief in the efficacy of exercise, lack of time and information. High-quality evidence indicates that land-based therapeutic exercise provides short-term benefit that is sustained for at least two to six months after cessation of formal treatment in terms of reduced knee pain, and moderate-quality evidence shows improvement in physical function among people with knee OA. The magnitude of the treatment effect would be considered moderate (immediate) to small (two to six months) but comparable with estimates reported for non-steroidal anti-inflammatory drugs. Confidence intervals around demonstrated pooled results for pain reduction and improvement in physical function do not exclude a minimal clinically important treatment effect. Since the participants in most trials were aware of their treatment, this may have contributed to their improvement. Despite the lack of blinding we did not downgrade the quality of evidence for risk of performance or detection bias. This reflects our belief that further research in this area is unlikely to change the findings of our review.

Background Chronic musculoskeletal pain (CMP) is a major health problem, accounting for approximately one-quarter of general practice (GP) consultations in the United Kingdom (UK). Exercise and physical activity is beneficial for the most common types of CMP, such as back and knee pain. However, poor adherence to exercise and physical activity may limit long-term e ectiveness. Objectives To assess the e ects of interventions to improve adherence to exercise and physical activity for people with chronic musculoskeletal pain. Search methods We searched the trials registers of relevant Cochrane Review Groups. In addition, we searched the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, CINAHL, AMED, PsycINFO, Science Citation Index and Social Science Citation Index and reference lists of articles to October 2007. We consulted experts for unpublished trials. Selection criteria Randomised or quasi-randomised trials evaluating interventions that aimed to improve adherence to exercise and physical activity in adults with pain for three months and over in the axial skeleton or large peripheral joints. Data collection and analysis Two of the four authors independently assessed the quality of each included trial and extracted data. We contacted study authors for missing information. Main results We included 42 trials with 8243 participants, mainly with osteoarthritis and spinal pain. Methods used for improving and measuring adherence in the included trials were inconsistent. Two of the 17 trials that compared di erent types of exercise showed positive e ects, suggesting that the type of exercise is not an important factor in improving exercise adherence. Six trials studied di erent methods of delivering exercise, such as supervising exercise sessions, refresher sessions and audio or videotapes of the exercises to take home. Of these, five trials found interventions improved exercise adherence. Four trials evaluated specific interventions targeting exercise adherence; three of these showed a positive e ect on exercise adherence. In eight trials studying self-management programmes, six improved adherence measures. One trial found graded activity was more e ective than usual care for improving exercise adherence.
