Objective. With the constant development of medical technology, the medical-nursing combination mode targeted at elderly chronic diseases has been proposed, and based on this, the study on the nursing mode of elderly chronic diseases under the background of medical-nursing combination was conducted. Methods. Total of 100 elderly patients with chronic diseases who were admitted to our hospital from April 2019 to April 2020 were selected and divided into a control group and an experimental group by lot drawing, with 50 patients in each group. Continuous nursing was conducted to patients in the control group, and continuous nursing combined with comfort nursing was performed to patients in the experimental group, so as to compare their nursing effective rate, satisfaction with nursing, self-care ability at 6 months after nursing, quality of life (QLI) scores, and blood glucose, pressure, and lipid levels at various periods. Results. Compared with the control group, the experimental group obtained significantly improved nursing effective rate, satisfaction with nursing, Exercise of Self-Care Agency (ESCA) scale scores, QLI scores, and lower blood glucose, pressure, and lipid levels at various periods 6 months after nursing, with statistical significance ( P all < 0.05). Conclusion. Under the context of medical-nursing combination, the continuous nursing intervention combined with comfort nursing intervention is conductive to promote the self-care ability and nursing effect of elderly patients with chronic diseases and obviously improve their quality of life and psychological state.

Objectives Continuity of care (CoC), entailing consistent, coherent, and integrated healthcare delivery by healthcare providers throughout a patient's lifetime, is multifaceted, encompassing relational, informational, and managerial dimensions. This study delves into the prevailing consultation preferences, continuity of care, and influential determinants among Chinese patients requiring blood glucose management, with the aim of optimizing healthcare consultations and enhancing continuity of care. Methods Questionnaires were disseminated across multiple digital health platforms, yielding a total of 548 valid responses. Employed analytical methods included descriptive statistical analysis, scale reliability assessment, chi-square tests for multiple response frequency cross-tabulation, independent sample t-tests, one-way ANOVA, Pearson and Spearman correlation analyses, quantile regression modeling, and multiple linear regression, all executed through IBM SPSS25. Results Approximately 58.21% of participants underscored the importance of a physician familiar with their comprehensive medical history, while 58.03% prioritized physicians who take the time to listen. Conversely, 41.7% and 40.0% of participants, respectively, reported lacking access to physicians exhibiting these characteristics during actual consultations. Individuals with suboptimal quality of life or medication adherence reported lower CoC than their counterparts (p < 0.01). Continuity of care was significantly higher among those with access to a familiar physician (p < 0.01). Regarding online health consultations, frequent users exhibited higher CoC compared to infrequent or non-users (p < 0.01). The influence of four variables - quality of life, medication adherence, access to a familiar physician, and frequency of online health consultations - on continuity of care was statistically significant at the quantile point. Conclusion This research offers critical insights for healthcare practitioners and policy designers to bolster continuity of care. Factors such as diminished quality of life, inadequate medication adherence, absence of a familiar physician, and infrequent or non-existent online health consultations potentially contribute to low continuity of care.

Background and Objective. Chronic obstructive pulmonary disease (COPD) features high morbidity. Different nursing interventions can relieve patient’s burden. This investigation quantitatively assessed the impact of transitional care (TC) on self-management of patients with acute exacerbation of COPD based on a questionnaire survey. Methods. Clinical information of 78 COPD patients treated with TC (intervention group) or routine care (control group) in Shanghai Pulmonary Hospital during March 2019 and August 2020 was gathered. Patients were followed up for 3 months after discharge. The intervention group ( n = 39 ) was subjected to a TC plan for 3 months to help patients and their family caregivers for self-management of COPD. TC was provided by specially trained nurses, and patients were supported by standardized tools. Nursing measures in the control group ( n = 79 ) included transitional support for 30 d after hospital discharge. In this way, patients were guaranteed to follow discharge plans and transit to outpatient nursing. Patient’s anxiety and depression symptoms, sleep quality, survival quality, mobility, and life quality at admission and after 3 months of discharge were assessed by Hospital Anxiety and Depression Scale, Pittsburgh Sleep Quality Index, Quality of Life Scale Abbreviated Version, Activity of Daily Life Scale, St. George’s Respiratory Questionnaire, and COPD Assessment Test. Results. Except for anxiety and depression, patient’s sleep quality, survival quality, mobility, and life quality in two groups were significantly improved. Moreover, average change of total CAT score during 3 months of intervention was -5.44, while that in the control group was -1.74 ( p = 0.011 ). Improvement of survival quality of patients in the intervention group ( p = 0.001 ) was markedly greater than that in the control group ( p = 0.016 ). Conclusion. Altogether, TC based on quantification by questionnaire survey is beneficial to COPD patient’s life quality and self-management.
