Effects of 25-hydroxyvitamin D level and its change on parathyroid hormone in premenopausal Chinese women

Abstract: We conclude that vitamin D deficiency is common in young women in Hong Kong. The cross-sectional analysis indicates that optimal 25(OH)D is >40 nmol/L, and the longitudinal data is consistent with a higher optimal value which is not defined in this study's results.

“…In Chinese premenopausal women, a serum vitamin D level of 40 nmol/l was found to be the optimum level to “suppress” PTH. 25 In an older study by Lips et al, 26 a level of 30 nmol/l was enough to suppress PTH level in elderly subjects. Interestingly, in Finnish adolescent girls, it was observed also that at levels of 40 nmol/l or above, PTH would start to be suppressed.…”

“…80,81 Obesity, however, is associated with lower levels of 25-OHD and a reduced dose response to oral vitamin D or UV radiation. 7,12 As it may take up to 2-5 months for serum levels of 25-OHD to plateau, 80,82 retesting should not take place before 3 months. There appears to be little difference in 25-OHD values whether the total amount of vitamin D is given as a daily, weekly or monthly dose, with no clear indication that one dosing schedule is more effective than another.…”

“…While vitamin D supplementation is needed to achieve a desired threshold of serum 25(OH)D concentrations among deficient individuals, there is no universally accepted threshold at which initiating vitamin D supplementation would achieve the greatest impact. In Victoria, Australia for those whose serum levels are 50 nmol/L and below recommendations are to supplement with higher dose 50000 IU per month and serum 25(OH)D to be rechecked after 3 months and then at 12 months [ 6 ] as it takes 3–5 months for serum 25(OH)D to plateau [ 14 , 15 ].…”
