Background Persons affected by leprosy usually prefer to hide their condition and often come to the health facility after visible Grade 2 disabilities start to appear. They are unaware of the diagnosis or of the subsequent consequences and decline to seek care at the early stage of the disease progression. This study aims to describe the trajectory of this illness and the patterns of care seeking by leprosy patients in Bangladesh. Methods This qualitative study relied on in-depth interviews in Nilphamri district which is known to be a leprosy endemic district in Bangladesh. Persons affected by leprosy over 18 years of age with a Grade 2 disability were selected for interview. A total of thirteen respondents were interviewed. Thematic analysis was used to interpret the data to identify themes, explore emerging patterns, and examine commonalities and contradictions. Results Lack of awareness in the general population about leprosy and a poor referral system appeared to be the major barriers to seeking health care at an early stage of disease progression. Persons affected by leprosy remain hidden for a long time because of its slow progression, leading to the development of more devastating disabling conditions. Conclusion Programmes to create awareness about leprosy may help in motivating people to access appropriate health care in a timely manner, and comply with the treatment requirements and recommendations. People with a level II leprosy disability have limited functions in society. In Nagan Raya District, there is a 41% leprosy endemic area with a 5% level II disability rate. The goal of this research was to prove the relationships of traditional medication, stigma, and leper's family support to level II leprosy disability. This research used mixedmethodsand a case-control design with 84 respondents42 case respondents (level II leprosy disability) and 42 control respondents (level 0 leprosy disability)chosen by consecutive sampling under some inclusion and exclusion criteria. Data collection used the in-depth interview method. This study used bivariate data analysis with the chi-square test and multivariate data analysis with the logistic regression test and the Miles and Huberman's model. The results of the chi-square analysis indicated that traditional medication had p value=0.014 (OR: 2.3; 95% CI: 0.2-7.7), stigma p=0.04 (OR:3.6;95% CI: 1.5-8.9), and family's support p=0.192 (OR:2.0; 95% CI: 0.7-5.8). The results of the logistic regression analysis showed that stigma was a risk factor for leprosy level II disability with p value=0.006, (OR=4.9; 95% CI: 1.6-15.1) and probability of 99.96%. Based on the interview results, lepers were feeling uncomfortable with the society's not acceptingthem in a good way. This was especially the case with khanduri(feast) when it came down to food in that the society assumed lepers would transmit the infectious, disgusting disease. The Nagan Raya District Health Office is expected to encourage the societyto accept lepers and provide information regarding the process of leprosy transmission so as to appease the community's fearand to prevent further disability.

Mental health, stigma, and neglected tropical diseases: A review and systematic mapping of the evidence

Recent years have seen an increase in recognition of the important impact that mental health, wellbeing, and stigma have on the quality of life of people affected by neglected tropical diseases (NTDs), including the publication of global normative guidance and policy frameworks. However, systematic collation of the evidence that can guide greater clarity of thinking for research and practical application of effective interventions is lacking. We used systematic mapping methodology to review the state of the evidence around mental health, stigma, and NTDs in low- and middle-income countries, applying a simple theoretical framework to explore intersections between these areas. We built on existing reviews on the links between each domain, bringing the reviews up to date, across the NTDs identified by the WHO (minus recent additions). After systematic searching of major databases, and exclusions, we identified 190 papers. Data extraction was done to inform key topics of interest, namely, the burden of mental distress and illness/stigma associated with NTDs, the mechanisms by which NTDs add to mental distress and illness/stigma, how mental distress and illness/stigma affect the outcome and treatment of NTDs, and efficacy of interventions to address these domains. We also document the recommendations given by the authors of included studies for research and interventions. We found that there has been a substantial increase in research, which remains very heterogeneous. It was dominated by skin conditions, especially leprosy and, less so, lymphatic filariasis. Few studies had a comparative and even fewer had an intervention design. Our findings were however consistent with existing reviews, pointing to a high prevalence of mental conditions, substantially mediated by stigma and exclusion and a lack of sufficient access to support for mental wellbeing in programmes, despite the existence of effective interventions. These interventions cut across mental health services, stigma reduction, community engagement, and empowerment of people affected. We conclude that the evidence justifies increased investment in practical and integrated interventions to support the wellbeing of people affected by NTDs but that there remains a need for implementation research of consistent quality, and basic science around the impact of mental health interventions on NTD outcomes (including on elimination efforts) needs to be strengthened.
