Effectiveness of the macrolide clarithromycin in the treatment ofMycobacterium avium complex infection in HIV-infected patients

Abstract: In a randomized double-blind study, nine mycobacteremic patients with AIDS-related disseminated Mycobacterium avium complex (MAC) infection received clarithromycin or placebo in addition to a basic regimen that included isoniazid, ethambutol and clofazimine. All four patients receiving clarithromycin showed blood culture conversion and clinical response. Of the five patients treated without clarithromycin, two showed resolution of mycobacteremia and clinical response, while another two died without having show… Show more

“…Even if there are rare reports on the use of INH in human MAC infections (2,29), its administration as a therapeutic adjunct is discouraged (11,20) and no animal models have been extensively evaluated. Considering that the mechanisms of action of this drug are not yet fully elucidated (1), it could be interesting to investigate the activity of INH, in combination with other antimicrobial agents, such as CLA-AMI, in appropriate animal models.…”

“…A double-blind clinical trial showed that within 6 weeks, monotherapy with clarithromycin produced a dramatic decrease in the number of viable bacteria in the blood of patients with AIDS and disseminated MAC infection (3). However, it was simultaneously noted that monotherapy with clarithromycin can lead to drug resistance (21).…”

“…Clarithromycin is a recently approved 14-membered macrolide with increased stability in acid and improved pharmacokinetics, including the appearance of a microbiologically active metabolite in humans (8). Clarithromycin possesses broad-spectrum antimicrobial activity, inhibiting a range of gram-positive and gram-negative organisms, some anaerobes, and atypical pathogens (1,3,14,16,25), in many cases with greater in vitro activity than erythromycin.…”
