Effectiveness of symptom validity measures in identifying cognitive and behavioral symptom exaggeration in adult attention deficit hyperactivity disorder

Abstract: This study examines the effectiveness of symptom validity measures to detect suspect effort in cognitive testing and invalid completion of ADHD behavior rating scales in 268 adults referred for ADHD assessment. Patients were diagnosed with ADHD based on cognitive testing, behavior rating scales, and clinical interview. “…Psychostimulants are frequently prescribed to enhance classroom learning and are increasingly sought out by individuals without ADHD for the same reason. Indeed, stimulants also enhance learning in normal populations (Rapoport et al 1980;Rapoport and Inoff-Germain 2002;Marshall et al 2010). “…Those looking to obtain a prescription for stimulant medications know which symptoms to report to their physicians (Kadison, 2005). In recent studies of adults being assessed for ADHD, between one-fifth and one-half of all patients are believed to have exaggerated their symptoms (Marshall et al, 2010;Sullivan, May, & Galbally, 2007). Given the ease with which symptoms are faked, prescriptions for stimulant medications can be easy to obtain.…”
