Key Clinical MessageA successful management of a mandibular recession with creeping attachment using free gingival graft. This case highlights the “wait‐and‐see” approach in the treatment plan, emphasizing the root coverage.AbstractThis clinical report describes the case of a 23‐year‐old female patient with gingival recession of the mandibular left central incisor. The patient's symptoms included hypersensitivity and cosmetic improvements. After thorough examination, the patient was diagnosed with plaque‐induced gingivitis on a reduced periodontium with acquired mucogingival deformities. Gingival recession was classified as Miller class II or Cairo Recession Type 1. The treatment plan consisted of a nonsurgical phase, followed by a surgical phase using a free gingival graft. The surgical procedure involved harvesting the graft from the palatal mucosa and placing it in the recipient site. Post‐operative care and instructions were provided and a comprehensive follow‐up schedule was established. At the 12‐month evaluation, the patient exhibited healthy periodontal conditions with creeping attachment and complete root coverage. The success of the treatment approach demonstrates its effectiveness in addressing hypersensitivity and cosmetic concerns, while improving periodontal health. Factors influencing the achievement of creeping attachment are discussed, including recession width, graft position, bone support, tooth position, patient hygiene, graft thickness, and root surface treatment.

Introdução: A hipersensibilidade dentinária é uma dor aguda e rápida e acomete diversos pacientes tendo uma etiologia multifatorial e entre as principais causas cita-se o uso de aparelho ortodôntico que também é um fator de risco para recessão gengival (RG), especialmente, em pacientes com fenótipo periodontal delgado. Objetivo: o presente trabalho tem como objetivo relatar um caso de RR múltiplo na região látero-inferiorutilizando enxerto autógeno em área deLCNCs.Relato de Caso: Paciente do sexo masculino, 32 anos de idade, não tabagista, nem etilista e sem alterações sistêmicas e sem alergias prévias foi encaminhado para uma clínica odontológica de um curso em periodontia, na cidade de Belém, norte do Brasil com a queixa principal de HD na região látero-inferior. No exame clínico verificou-se RG tipo 1 (RT1) múltiplas, nos dentes 43, 44 (presença de lesão cervical não cariosa) e 45, presença de fenótipo periodontal fino e festonado, sem perdas ósseas interproximais, ausência de sangramento a sondagem, biofilme e cálculo dentário.O tratamento indicado foi o recobrimento radicular pela técnica de deslize coronal associado ao enxerto autógeno para melhoria do nível clínico de inserção e aumento da faixa de mucosa queratinizada com MDA fixada com suturas. Conclusão: Diante do exposto, verificamos que os resultados clínicos relatados neste trabalho indicaram um ótimo resultado clínico para aumento da faixa de mucosa queratinizada, ganho clínico de inserção e resolução da HD.

A dysbiotic microbial community whose members have specific/synergistic functions that are modulated by environmental conditions, can disturb homeostasis in the subgingival space leading to destructive inflammation, plays a role in the progression of periodontitis. Filifactor alocis, a gram‐positive, anaerobic bacterium, is a newly recognized microbe that shows a strong correlation with periodontal disease. Our previous observations suggested F. alocis to be more resistant to oxidative stress compared to Porphyromonas gingivalis. The objective of this study is to further determine if F. alocis, because of its increased resistance to oxidative stress, can affect the survival of other ‘established’ periodontal pathogens under environmental stress conditions typical of the periodontal pocket. Here, we have shown that via their interaction, F. alocis protects P. gingivalis W83 under H2O2‐induced oxidative stress conditions. Transcriptional profiling of the interaction of F. alocis and P. gingivalis in the presence of H2O2‐induced stress revealed the modulation of several genes, including those with ABC transporter and other cellular functions. The ABC transporter operon (PG0682–PG0685) of P. gingivalis was not significant to its enhanced survival when cocultured with F. alocis under H2O2‐induced oxidative stress. In F. alocis, one of the most highly up‐regulated operons (FA0894–FA0897) is predicted to encode a putative manganese ABC transporter, which in other bacteria can play an essential role in oxidative stress protection. Collectively, the results may indicate that F. alocis could likely stabilize the microbial community in the inflammatory microenvironment of the periodontal pocket by reducing the oxidative environment. This strategy could be vital to the survival of other pathogens, such as P. gingivalis, and its ability to adapt and persist in the periodontal pocket.
