Effectiveness of Self-cut vs Mesh-Kit Titanium-Coated Polypropylene Mesh for Transvaginal Treatment of Severe Pelvic Organ Prolapse

Abstract: IMPORTANCETransvaginal mesh (TVM) can increase the durability of vaginal surgical procedures for pelvic organ prolapse (POP) and may be indicated in certain situations despite concerns about mesh-related complications. In addition, the expense of commercial mesh kits has limited their use.The effectiveness, safety, and cost of a self-cut mesh procedure compared with a commercial mesh-kit procedure for the surgical treatment of women with POP is unclear.OBJECTIVE To assess the 1-year effectiveness and safety of… Show more

“…One particularly promising aspect of using sheep as a model is that some research groups have successfully transitioned from preclinical trials in sheep to clinical trials in humans. A research group in China carried out initial investigations in the ovine model and continued to human clinical trials using their titanium-coated PP mesh, TiLOOP [ 43 ]. Note that this was the only animal study of a novel implant for POP identified to have been carried on into human clinical trials.…”

“…The studies took place in 11 countries ( Figure 2 ). France (17.8%) was the country with the most significant number of studies [ 13 , 14 , 15 , 16 , 17 ], followed by the United Kingdom (14.3%) [ 18 , 19 , 20 , 21 ], Italy (14.3%) [ 7 , 22 , 23 , 24 ], Brazil (14.3%) [ 25 , 26 , 27 , 28 ], Holland (10.8%) [ 29 , 30 , 31 ], China (10.8%) [ 32 , 33 , 34 ], the United States of America (3.6%) [ 35 ], Germany (3.6%) [ 36 ], Spain (3.6%) [ 8 ], Denmark (3.6%) [ 37 ], and Iran (3.6%) [ 38 ].…”

“…We included 7 studies [ 7 , 8 , 13 , 22 , 23 , 35 , 36 ] that only involved laparoscopic treatment, 3 studies regarding transvaginal mesh treatment [ 17 , 33 , 34 ], and 18 mixed studies (mesh laparoscopic and vaginal mesh, vaginal mesh and native tissue, biologic xenograft and vaginal mesh, laparoscopic mesh and robotic mesh, and laparoscopic mesh and abdominal mesh) [ 14 , 15 , 16 , 18 , 19 , 20 , 21 , 24 , 25 , 26 , 27 , 28 , 29 , 30 , 31 , 32 , 37 , 38 ].…”

“…[17][18][19] Third is the Chinese academic research group represented by Zhu Lan, whose main research interests are the analysis of risk factors for pelvic organ prolapse and the clinical translation of basic and applied anatomical research on pelvic floor dysfunction disorders, [20] In recent years, Professor Zhu Lan main research interests include the study of the mechanism of pelvic organ prolapse, [21] the exploration of the "gold standard" surgical procedure for apical prolapse, and the standardization of the registration of complications after implantation of synthetic mesh pelvic floor reconstruction. [22,23] The number of articles published in China is the second largest in the field of pelvic floor reconstruction for the treatment of POP, indicating significant research efforts and contributions. However, the lack of research institutions and authors with mediated centrality >0.1 suggests that the influence of pelvic floor reconstruction in China needs further enhancement.…”
