Effectiveness of non invasive external pelvic compression: a systematic review of the literature

Abstract: IntroductionPelvic fractures might carry a significant risk of bleeding. A wide variety of pelvic binders together with pelvic sheets are available and offer an adjunct to the initial management of poly-trauma patients with pelvic injuries. These devices are collectively referred to as pelvic circumferential compression devices (PCCDs). The aim of this study was to review the literature for evidence pertinent to the efficacy and safety of PCCDs.MethodsUsing the PRISMA guidelines a systematic search on PubMed, … Show more

“…As the long-term effects of pelvic binder remain unclear at present, including the potential risk of soft tissue complications from prolonged compression [45], the general recommendation is to remove pelvic binders as soon as physiologically justifiable [26], and to consider replacing binders by external pelvic fixation.…”

“…The ATLS protocol has been shown to improve doctors’ knowledge and performance regarding acute management of poly-trauma patients [ 5 ]. Decreasing the intra-pelvic volume in patients with pelvic injury is the idea behind the application of a PCCD [ 6 ]. To achieve maximum mechanical stability in a disrupted pelvic ring it is recommended that PCCD be applied at the level of the greater trochanters [ 6 ].…”

“…Pelvic binders provide circumferential compression of the bony pelvis, providing emergency stabilisation of a pelvic fracture and theoretically reducing blood loss. Observational studies suggest a survival benefit associated with their use (Bakhshayesh et al, 2016). The risks associated with binder application for short periods are minimal, but prolonged use should be avoided to prevent pressure-related complications.…”
