Oral mucositis is a debilitating complication of chemotherapy, characterized by erythema, ulcers and oedema of the oral mucosa. This review aimed to evaluate the efficacy of Photobiomodulation in the treatment of oral mucositis using meta-analysis and trial sequential analysis, and also to assess the quality of the results by Grading of Recommendations, Assessment, Development and Evaluation (GRADE). A comprehensive search of three databases, including Embase, Medline and Central, was performed to identify randomized controlled trials studying the efficacy of Photobiomodulation in the treatment of cancer chemotherapy-induced oral mucositis. The primary outcome was reduction in the severity of oral mucositis. Secondary outcomes were pain relief, duration of oral mucositis and adverse effects. The meta-analysis was performed using the random-effects model, and random errors of the meta-analyses were detected by trial sequential analysis. A total of 6 randomized controlled trials with 398 participants were included in our analysis. Photobiomodulation significantly reduced the severity of oral mucositis when compared to sham radiation (RR 0.43, 95% CI 0.20 to 0.93; p < 0.05). Sensitivity analysis by excluding trials with high risk of bias reiterated the robustness of our results (RR 0.28, 95% CI 0.16 to 0.48). Trial sequential analysis illustrated that the evidence from the meta-analysis was conclusive. The result of the meta-analyses with trial sequential analysis illustrated that Photobiomodulation is an effective therapeutic intervention for the treatment of oral mucositis, and the evidence gathered can be considered conclusive with a moderate level of certainty according to GRADE. Further trials are recommended to standardize the laser parameters required for the optimal effect.

Capecitabine-induced hand-foot syndrome (HFS) is common in clinical practice.There are many regimens used to prevent HFS. However, the most effective preventive regimen has not yet been identified. Thus, we conducted a network metaanalysis to investigate the best preventive regimen for HFS. The PRISMA-NMA guidelines were used in this study. The PubMed, Cochrane, and Embase databases were searched. The main endpoint was set as HFS of National Cancer Institute grade 2 or more. We included only randomized control trials. The P-score was used to rank the regimens. Among all the regimens, topical silymarin had the best preventive ability compared with the placebo (OR: 0.08; 95% CI: 0.01-0.71). The other identified effective regimen included pyridoxine (400 mg) and celecoxib; compared with the placebo, the odds ratio was 0.27 (95% CI: 0.08-0.91) and 0.41 (95% CI: 0.18-0.95), respectively. Topical silymarin is the most useful regimen for preventing capecitabine-induced HFS.

La mucositis oral (MO) afecta a niños y jóvenes con tratamiento oncológico, donde se ha sugerido la terapia láser de baja potencia (o LLLT por sus siglas en inglés) como método preventivo. El objetivo de este trabajo fue analizar mediante una sinopsis, los efectos de la LLLT sobre la prevención y tratamiento del cuadro clínico de MO en población pediátrica oncológica tratada con radioterapia y/o quimioterapia. Se realizó una búsqueda electrónica de revisiones sistemáticas publicadas entre 2018 y 2021 en la base de datos Medline, metabuscador Epistemonikos y registro Google Académico. La estrategia de búsqueda fue "low-level light therapy AND oral mucositis AND pediatric patient". Se encontraron 273 artículos, analizándose dos revisiones sistemáticas. La LLLT mostró disminuir el riesgo de MO (OR = 0.50, IC 95%, 0.29 a 0.87) mejorando el cuadro clínico al séptimo día (RR= 0.90, IC 95%, 0.46 a 1.78) con pocos eventos adversos. La LLLT parece disminuir el riesgo de MO y la gravedad del cuadro, aunque se requieren de más estudios para identificar una dosis óptima.
