Effectiveness of exercise interventions on physical function in community-dwelling frail older people: an umbrella review of systematic reviews

Abstract: Interventions for pre-frail and frail older adults should include multi-component exercises, including in particular resistance training, as well as aerobic, balance and flexibility tasks. Future research should adopt a consistent definition of frailty and investigate the effects of other types of exercise alone or in combination with nutritional interventions so that more specific recommendations can be made.

“…Exercise training has been demonstrated to be beneficial in improving frailty symptoms and cognitive function, in addition to being cost effective (26,27). The evidence corroborates with the existing notion that exercise has the potential to reverse frailty by improving physical function (28).…”

“…Thus, we developed a multicomponent exercise program based on the combination of the available evidence. Firstly, the components of aerobic exercise, progressive resistance training, balance and flexibility were selected as it addresses the frailty criterion of weakness, slow motor performance, low physical activity, and fatigability (26,37). Next, we benchmarked the evidence in an umbrella systematic review of systematic reviews (total of 58 RCTs) evaluating the effectiveness of exercise-based interventions among those with pre-frailty and frailty (26).…”

“…An assessment of PF provides a basis for an early evaluation of functional decline in older persons, whose PF ranges from vigorous to frail, and can guide geriatric treatment strategies (2). Furthermore, as an outcome measure, PF is a vital component of studies comparing groups or evaluating the effect of different interventions on functional status in older adults (3,4).…”

“…The majority of previous studies are based on short-term interventions among either healthy older adults or among older adults undergoing rehabilitation for specific clinical conditions [10,12]. More recently, the evidence on the effectiveness of SBT on community-dwelling frail population has been strengthening [15,16]. Nonetheless, among the longterm interventions, the intervention length only seldom has exceeded 1 year [9].…”
