AbstrakEmpati merupakan dasar dari hubungan terapetik antara dokter dan pasien. Pengukuran empati berdasarkan persepsi pasien menggunakan kuesioner The Consultation and Relational Empathy (CARE) telah digunakan secara luas dan telah diterjemahkan ke dalam beberapa bahasa di dunia. Tujuan dari penelitian ini adalah menguji psikometri dari kuesioner CARE versi bahasa Indonesia, mengukur rerata empati dokter dan menguji perbedaan nilai rerata empati antara kategori usia, jenis kelamin, tingkat pendidikan, penghasilan, jumlah keluhan, penyakit kronis, jenis kelamin dokter dan lokasi fasilitas kesehatan primer.Data dikumpulkan dari 336 pasien yang memeriksakan diri ke 21 dokter di 6 fasilitas kesehatan primer. Lokasi penelitian berada di kota dan kabupaten Malang. Validitas konstruk kuesioner diperiksa dengan korelasi Pearson sedangkan reliabilitas kuesioner diukur dengan metode Cronbach's alpha. Uji beda rerata nilai empati per karakteristik pasien, dokter dan lokasi fasilitas kesehatan primer diukur menggunakan independent t test dan ANOVA. Analisis data terhadap validitas kuesioner CARE berbahasa Indonesia menunjukkan nilai corrected item-total score correlations dalam rentang 0,623-0,694 dengan nilai Cronbach's alpha = 0,902. Rerata empati dokter bervariasi diantara rentang 27 hingga 50 dengan rerata total 40,69. Studi ini menyimpulkan bahwa kuesioner CARE versi bahasa Indonesia dapat digunakan untuk mengukur empati dokter di layanan primer karena memiliki validitas dan reliabilitas yang baik. Nilai rerata empati dokter secara keseluruhan berada pada rentang rata-rata (average). Tidak terdapat perbedaan nilai empati pada hampir semua karakteristik pasien, kecuali pada parameter penghasilan dan lokasi fasilitas kesehatan primer.Kata Kunci: empati, Malang, kuesioner CARE, reliabilitas, pelayanan primer DOCTOR'S EMPATHY IN PRIMARY CARE: MEASUREMENT USING THE INDONESIAN VERSION OF CONSULTATION AND RELATIONAL EMPATHY (CARE) QUESTIONNAIREEmpathy is considered as a basic component of the therapeutic relationships. This study were aimed to examine psychometric properties ofan Indonesian CARE questionnaire, and to study the total mean score and differences in empathy scores between ages, gender of patient, educational level, income, number of complaints, chronic condition, gender of doctor and the location of primary care provider.Data were collected from 336 primary care patients who attending to 21 doctors in 6 primary health care providers located in Malang region. The construct validity of the Indonesian CARE was assessed with Pearson correlation while the reliability was assessed with Cronbach's alpha. Recent studies have revealed a drop in the ability of physicians to empathize with their patients. It is argued that empathy training needs to be provided to both medical students and physicians in order to improve patient care. While it may be true that empathy would lead to better patient care, it is important that the right theory of empathy is being encouraged. This paper examines and critiques the prominent explanation of empathy being used in medicine. Focusing on the component of empathy that allows us to understand others, it is argued that this understanding is accomplished through a simulation. However, simulation theory is not the best explanation of empathy for medicine, since it involves a limited perspective in which to understand the patient. In response to the limitations and objections to simulation theory, interaction theory is presented as a promising alternative. This theory explains the physicians understanding of patients from diverse backgrounds as an ability to learn and apply narratives. By explaining how we understand others, without limiting our ability to understand various others, interaction theory is more likely than simulation theory to provide better patient care, and therefore is a better theory of empathy for the medical field.

(Kachru, 1992), including countries such as India, Pakistan and Sri Lanka, where a different variety of English is spoken (AMC, 2009(AMC, , 2011(AMC, , 2012a(AMC, , 2012b.In addition to the challenges of working in an unfamiliar medical system, these doctors can also face considerable difficulties in daily communications with patients, families, and colleagues (Hall, Keely, Dojeiji, Byszewski, & Marks, 2004;McDonnell & Usherwood, 2008;Tipton, 2005). In addition to the more obvious language difficulties with vocabulary, grammar, pronunciation, and an understanding of accents and colloquialisms, they can also struggle with the less salient but vitally important interpersonal features of language use, such as how to build rapport and show empathy (Hall et al., 2004;McDonnell & Usherwood, 2008;Pilotto, Duncan, & Anderson-Wurf, 2007). Moreover, many may not be familiar with the demands of the patientcentred models of care expected in developed medical environments in Canada and Australia, but less common in developing countries where medical facilities are very stretched (Dahm, 2011b;Khalil & Bhopal, 2009).In this article we focus on the communication challenges facing doctors who trained in medical environments very different from those found in Canada and Australia using a language other than English, in order to inform communications training designed specifically for doctors from language backgrounds other than English, and to illustrate how a close analysis of professional discourse can be transferred to ESL classes preparing for work environments beyond the medical world. We draw on clinical role-plays performed by practicing locally trained native English-speaking (NES) doctors and nonnative English-speaking (NNES) IMGs to identify the communication features of the kind of patient-centred approach to medical communication that will be expected of them. Although specific features and approaches to communication in Canada and Australia likely differ in some minor respects, our aims are to highlight features that are relevant in both cultures and to illustrate how discourse data can be used to identify culturally appropriate ways of communicating in a medical setting in order to provide an accurate evidence base from which culturally appropriate medical communication courses for IMGs may be developed.
