Introducción: se considera como quemadura al daño que sufre la piel y tejidos frente a un estímulo caliente, se dividen dependiendo el daño, que va desde el 1er grado hasta el 4to grado. En Ecuador, se realizó un estudio en el Hospital Baca Ortiz de la ciudad de Quito donde existe una referencia que, de un total de 343 pacientes pediátricos con quemaduras. Las cicatrices por quemaduras siguen siendo un problema crónico después de lesiones por quemaduras, y las cicatrices sintomáticas se desarrollan con mayor frecuencia después de quemaduras profundas o de gran superficie que tardan más de tres semanas en sanar. Se consideró a la rehabilitación era un tratamiento posterior, pero ahora se ha demostrado que es eficaz y beneficiosa para una variedad de enfermedades y trastornos y se reconoce como un aspecto esencial del buen pronóstico funcional de los pacientes. Objetivo: Revisar la evidencia científica disponible sobre la intervención fisioterapéutica temprana para la recuperación y rehabilitación del paciente quemado. Metodología: Se realizó una revisión narrativa de la literatura desde perspectiva profesional Fisioterapeuta – Psicología. La revisión bibliográfica de documentos en las bases de datos Scopus, Google Scholar y SciELO. Fueron seleccionaron un total de 20 artículos, en inglés y español, se emplearon los descriptores Fisioterapia, kinesioterapia, ejercicio y quemaduras. Discusión: Se proponen protocolos y pautas de tratamiento, desde las quemaduras leves hasta graves que requieren de hospitalización e intervención quirúrgica con un equipo de salud multidisciplinario para que el paciente tenga atención integral. Conclusión: Podemos sostener que el tratamiento de fisioterapia y rehabilitación en pacientes con quemaduras independiente del grado es oportuno y eficaz siempre y cuando tenga la valoración respectiva y el enfoque rehabilitador como puede ser recuperar rangos de movilidad o a su vez mejorar calidad de vida en los pacientes desde el punto de vista rehabilitación integral con equipo multidisciplinario. Background: Hand burns are involved in 80–90% of severe burn injuries. Even though hands correspond to a small total burn surface area (TBSA) of less than 5%, the loss of their functionality has a significant impact on the patient’s life. Studies that provide long-term results regarding hand functionality after hand burns are scarce. Therefore, this study aimed to investigate functional long-term results in a patient-centric approach regarding burn depth, unilateral and bilateral hand involvement, and (non-)isolated hand burns as potential influencing factors in patients with hand burns. Methods: We conducted a controlled cohort study of patients with burned hands treated at our department between 2005 and 2022. Healthy age-, sex-, and handedness-matched participants were used as controls. Data on the demographics, burn-related injuries, and treatments were collected. For a patient-centric approach, we used the Disabilities of the Arm, Shoulder, and Hand Questionnaire (Quick-DASH) and the Michigan Hand Questionnaire (MHQ) as patient-reported outcome measures for functional long-term evaluation, and the Patient and Observer Scar Assessment Scale (patient scale) for assessing long-term scar quality. Results: We enrolled 61 patients with 88 affected hands and 63 matched control participants. Up to 77.1% of the participants were male, with a mean age of 50.7 (±15.5) years and a follow-up of 8.1 (±4.7) years. The mean TBSA was 13.9% (±15.8), with 72.4% of the hands presenting with deep partial-thickness and full-thickness burns and most of the patients had only one hand affected (55.7%). The hand burn patients perceived significantly worse long-term functional scores in every domain of the MHQ as well as in the “overall function” and “work” of the Quick-DASH. Superficial hand burns negatively affected the two-handed activities of daily living (p = 0.013) and aesthetic appearance (p = 0.005) when both hands were involved. Isolated hand burns were associated with more difficulties in work performance (p = 0.03), whereas patients with bilateral hand involvement perceived more pain (p = 0.025). Conclusions: The patients with hand burns can achieve satisfactory long-term functional outcomes over time, but they do not reach the same long-term hand functionality as the healthy matched control group. Our study revealed that factors such as burn depth, unilateral or bilateral hand involvement, and (non-)isolated hand burns indeed have an impact on certain aspects of perceived long-term hand functionality.

Severe and common injuries involving burns to the hands and wrists can often lead to permanent loss of motion. The issue is exacerbated by the delicate nature of tendons and muscles in the hands, along with the formation of scar tissue. While rehabilitation exercises can help improve the range of motion, early-stage recovery requires additional tension on the affected areas. To address this concern, a novel project was initiated, aiming to develop a specialized splint for later-stage rehabilitation. This innovative splint allows users to carry out their daily tasks while wearing it, constantly applying a beneficial load on the wrist, hand, and digits to enhance range of motion. The development of the splint involved leveraging Fused Deposition Modelling (FDM) 3D printing and medically safe materials for the initial prototype. Finite Element Analysis (FEA) was employed to analyze the design. The process underwent iterative design improvements and parameter adjustments, ultimately resulting in the final prototype. The FEA analysis confirmed the strength and durability of the PLA components, while the TPU digit resistance bands were evaluated using a hyper-elastic model. As a result, the final design effectively applies tension to the digits without compromising day-to-day tasks’ usability and wearer’s comfort. Future iterations of the splint could focus on enhancing fastening methods, reducing brace movement during usage, creating various sizes to accommodate different arm/hand dimensions, and optimizing mass-manufacturing processes.
