Effectiveness of double tie-over dressing compared with bolster dressing

Abstract: BackgroundSkin grafting is a commonly performed operation in plastic and reconstructive surgery. The tie-over dressing is an effective technique to secure the grafted skin by delivering persistent downward pressure. However, if an additional dressing is required due to incomplete graft healing, the process of re-implementing the tie-over dressing may be frustrating for both patients and surgeons. Therefore, we introduce the double tie-over dressing, which readily allows for an additional tie-over dressing afte… Show more

“…A nonadherent dressing is placed on top, and only 1 strand from each knot is tied over, leaving the other strand for an additional tie-over dressing if needed after removing the first dressing. In a retrospective comparison of 69 double TOBs in which the first dressing was removed after 5 days, and 59 simple TOBs, skin grafts in the head and neck receiving the double-TOB required less time to heal 20 …”

“…In a retrospective comparison of 69 double TOBs in which the first dressing was removed after 5 days, and 59 simple TOBs, skin grafts in the head and neck receiving the double-TOB required less time to heal. 20 Recommendation: Although TOB is a reproducible method of fixation, its influence on graft take is equivocal. Alternatives are available for use: (i) peripheral sutures for small FSGs, (ii) foam sponge bolsters in topographically challenging and sensitive areas, and (iii) relook methods in higher-risk cases to facilitate early graft check.…”

“…The remaining three studies compared the tie-over dressing against “non-tie-over dressings” ( n = 71),[ 71 ] hydrocolloid dressings ( n = 62),[ 72 ] and double-tie over dressings ( n = 128). [ 73 ] No significant differences were found between groups.…”
